1.1 Follow the most up-to-date LEED, building code, electrical code, and fire code requirements that exist during the construction of this contract. The most up-to-date codes supersede any others printed within these specifications that may be out-dated at the time of construction.

SUBMITTAL PROCEDURES

1.3 SUBMITTALS: The submittals described below are those required and further described in other sections of the specifications. Other requirements pertaining to submittals are included in included in individual specification sections. Submittals required by the CONTRACT CLAUSES and other non-technical parts of the contract are not included in this section.
SD-01 Data
Submittals, which provide calculations, descriptions, or documentation regarding the work.
SD-06 Instructions
Pre-printed material describing installation of a product, system or material, including special notices and material safety data sheets, if any, concerning impedances, hazards, and safety precautions.
SD-07 Schedules
Tabular lists showing location, features, or other pertinent information regarding products, materials, equipment, or components to be used in the work.
SD-08 Statements
A document, required of the Contractor, or through the Contractor, from a supplier, installer, manufacturer, or other lower tier Contractor, the purpose of which is to confirm the quality or orderly progression of a portion of the work by documenting procedures, acceptability of methods or personnel, qualifications, or other verifications of quality.
SD-09 Reports
Reports of inspections or tests, including analysis and interpretation of test results. Each report shall be properly identified. Test methods used shall be identified and test results shall be recorded.

Statement signed by an official authorized to certify on behalf of the manufacturer of a product, system or material, attesting that the product, system or material meets specified requirements. The statement must be dated after the award of this contract, must state the Contractor's name and address, must name the project and location, and must list the specific requirements, which are being certified.
SD-14 Samples
Samples, including both fabricated and un-fabricated physical examples of materials, products, and units of work as complete units or as portions of units of work.
SD-18 Records
Documentation to record compliance with technical or administrative requirements.
SD-19 Operation and Maintenance Manuals
Data, which forms a part of an operation and maintenance manual.
PART 2 GENERAL
2.1 SUMMARY – NOT APPLICABLE
2.2 REFERENCES – NOT APPLICABLE
2.3 DEFINITIONS:
2.3.1 Submittal: A submittal is a package of project information, samples, schedules, certificates, etc., submitted to the Contracting Officer for Government review.
2.3.2 Deviation: A specific submittal where an item is identified as not agreeing with the contract requirements and the Contractor is requesting a substitution or change.
2.3.3 Approval/Disapproval: Specific judgments reserved to the Government concerning a submittal's compliance with the plans and specifications.
2.4 PURPOSE: Submittals are directed for the convenience of the Government in reviewing the Contractor's planned approach and compliance with the requirements of the contract. It is also a mechanism whereby the Contractor may propose deviations, color choices, etc., at an early point in the contract where changes in approach will have less impact on the materials ordering process.
2.5 SUBMITTAL CLASSIFICATION: Submittals are classified as follows:
2.5.1 Government Approved (GA): Governmental approval is required for extensions of design, critical materials, deviations, equipment whose compatibility with the entire system must be checked, and other items as designated by the Contracting Officer.

2.5.2 Information Only (FIO): All submittals not requiring Government approval will be for information only. They are not considered to be "shop drawings" within the terms of 1.3.1 SUBMITTAL paragraph referred to above.
2.6 APPROVED SUBMITTALS: The approval of submittals by the Contracting Officer shall not be construed as a complete check, but will indicate only that the general method of construction, materials, detailing and other information are satisfactory. Approval will not relieve the Contractor of the responsibility for any error which may exist, as the Contractor under the CQC requirements of this contract, is responsible for the dimensions and design of adequate connections, details and satisfactory construction of all work. After submittals have been approved by the Contracting Officer, no resubmittal for the purpose of substituting materials or equipment will be given consideration unless accompanied by an explanation as to why a substitution is necessary.
2.7 DISAPPROVED SUBMITTALS: The Contractor shall make all corrections required by the Contracting Officer and promptly furnish a corrected submittal in the form and number of copies as specified for the initial submittal. If the Contractor considers any correction indicated on the submittals to constitute a change to the contract, notice shall be given promptly in writing to the Contracting Officer in accordance with the changes clause.
2.8 WITHHOLDING OF PAYMENT: Payment for materials incorporated in the work will not be made if required approvals have not been obtained.
PART 3 PRODUCTS – NOT APPLICABLE
PART 4 EXECUTION
4.1 GENERAL: Material submittals as required by Contract Clause 52.236-5, Material and Workmanship, and shop drawings as required by Contract Clause 52.236-21, Specifications and Drawings for Construction, are listed on AF Form 66, Schedule of Material Submittals, attached to this specification section. The Government may request submittals in addition to those listed when deemed necessary to adequately describe the work covered in the respective sections.
Units of weights and measures used on all submittals shall be the same used in the contract drawings. Each submittal shall be complete and in sufficient detail to allow ready determination of compliance with contract requirements. Prior to submittal, all items shall be checked and approved by the Contractor and each respective AF Form 3000 shall be signed and dated by the contractor certifying that the accompanying submittal complies with the contract requirements.
Proposed deviations from the contract requirements shall be clearly identified. Submittals shall include items such as: Contractor's, manufacturer's, or fabricator's drawings; descriptive literature including (but not limited to) catalog cuts, diagrams, operating charts or curves; test reports; test cylinders; samples; O&M manuals including parts list; certifications; warranties and other such required submittals.
4.2 SCHEDULING

4.2.1 Contractor shall submit Shop Drawings and Material Submittals within fourteen (14) calendar days after date of receipt of notice to proceed. Each submittal shall be scheduled and coordinated with the Progress Schedule in order to obtain Government approval prior to the acquisition of the material or equipment covered by the submittal. Fourteen (14) calendar days, exclusive of any mailing time, shall be allowed for Government review of submittals or resubmittals.
4.3 SUBMITTAL PROCEDURES:
4.3.1 All submittals shall be accomplished on and in accordance with instructions pertaining to AF Form 3000, Material Approval Submittal, except as otherwise directed herein. These forms will be furnished to the Contractor. This form shall be properly completed by filling out all the heading blank spaces and identifying each item submitted. Special care will be exercised to ensure proper listing of the specification paragraph and/or sheet number of the contract drawings pertinent to the data submitted for each item.
4.3.2 Forward all submittals directly to the Contracting Officer-Mr. Andrew Sorensen, 1655 Selfridge Ave, JBSA-Lackland, TX 78236.
4.3.3 Submittals shall be made in four (4) sets except as otherwise noted. One copy of each submittal will be returned to the Contractor.
4.4 DEVIATIONS: All requests for deviations shall be clearly identified by the Contractor on AF Form 3000. The Contractor shall set forth in writing the reason for any deviations and annotate such deviations on the submittal. The Government reserves the right to rescind inadvertent approval of submittals containing unnoted deviations.
4.5 GOVERNMENT APPROVED SUBMITTALS: Upon completion of review of submittals requiring Government approval (GA), the appropriate blocks on the AF Form 3000 will be completed by the Government, including signature(s) and dates, indicating approval or disapproval of the submittal. One (1) copy of the submittal and AF Form 3000 will be returned to the Contractor.
4.6 INFORMATION ONLY SUBMITTALS: Normally submittals for information only will not be returned. Government approval is not required on information only submittals. These submittals will be used for information purposes. The Government reserves the right to require the Contractor to resubmit any item found not to comply with the Contract. This does not relieve the Contractor from the obligation to furnish material conforming to the plans and specifications and will not prevent the Government from requiring removal and replacement if nonconforming material is incorporated in the work. This does not relieve the Contractor of the requirement to furnish samples for testing by the Government laboratory or check testing by the Government in those instances where the technical specifications so prescribe.

PROJECT MEETINGS
1.1 GENERAL
A. Design Conferences
1. Pre-Work Conference
a. As part of the Pre-Work Conference conducted after contract award, key representatives of the Government and the Contractor will review procedures specified herein and discuss the preliminary schedule as appropriate. The Contractor's Project Manager, Construction Quality Control Managers will attend this meeting.
The Contractor is encouraged to have an officer of the company at this conference.
This conference will be held at the location specified by the Contracting Officer's authorized representative. Minutes of the meeting will be prepared by the Government and signed by both the Contractor and the Contracting Officer. The minutes shall become a part of the contract file.
2. Scope Confirmation Meeting
a. After award of the contract and the Pre-Work Conference, the Contractor shall visit the site and conduct extensive interviews and problem solving discussions with the individual users and base personnel to acquire all necessary site information, review user options, and discuss user needs. The Contractor shall document all discussions.
Site visits and discussions shall be coordinated with the Contracting Officer's authorized representative.

B. Qualifications
1. The Contractor shall designate an authorized representative who shall be responsible for the preparation of all required project schedule reports.
C. Submittals
1. Government approval is required for submittals with a "g" designation; submittals having a "FIO" designation are for information only. When used, a designation following the "GA" designation identifies the office that will review the submittal for the Government.
The following shall be submitted in accordance with Section 01 30 00 SUBMITTAL

1.2 PRODUCTS – NOT USED
1.3 EXECUTION
A. GENERAL REQUIREMENTS: Use of the AF IMT 3064 will be the normal requirement for contractor construction schedule issued under this contract and shall include the required items, submittals, level of detail and contract requirements listed below for critical path method activities (Work Elements), in this paragraph C. through F and as detailed in the Instructions to Contractors on the reverse side of the AF IMT 3064, Form.
a. Critical Activities: The following activities can be provided by the Contractor at the Pre Construction Meeting:
1) Pre-final inspection.
2) Correction of punch list from pre-final inspection.
3) Final inspection.
4) Construction phasing.
b. Government Activities: Government and other agency activities that could impact progress shall be shown. These activities include, but are not limited to: approvals, environmental permit approvals by State regulators, inspections, and Notice to Proceed (NTP) for phasing requirements.
c. Responsibility: All activities shall be identified in the project schedule by the party responsible to perform the work. Responsibility includes, but is not limited to, the subcontracting firm, contractor work force, or government agency performing a given task. Activities shall not belong to more than one responsible party. The responsible party for each activity shall be identified by the Responsibility Code.

d. Work Areas: All activities shall be identified in the project schedule by the work area in which the activity occurs. Activities shall not be allowed to cover more than one work area. The work area of each activity shall be identified by the Work Area Code.
2. Scheduled Project Completion: The schedule interval shall extend from NTP to the Contract completion date.
a. Project Start Date: The schedule shall start no earlier than the date on which the NTP was acknowledged. Constraint of Last Activity: Completion of the last activity in the schedule shall be constrained by the contract completion date. Calculation on project updates shall be such that if the early finish of the last activity falls after the contract completion date, then the float calculation shall reflect a negative float on the critical path. The Contractor shall include as the last activity in the project schedule an activity called "End Project". The "End Project" activity shall have a "Late Finish" constraint date equal to the completion date for the project, and a zero day duration.
b. Early Project Completion: In the event the project schedule shows completion of the project prior to the contract completion date, the Contractor shall identify those activities that have been accelerated and/or those activities that are scheduled in parallel to support the Contractor's "early" completion. Contractor shall specifically address each of the activities noted in the AF 3065 at every project schedule update period to assist the Contracting Officer in evaluating the Contractor's ability to actually complete prior to the Contract period.
3. Interim Completion Dates: Contractually specified interim completion dates shall also be constrained to show negative float if the early finish date of the last activity in that phase falls after the interim completion date.
c. Start Phase: The Contractor shall include as the first activity for a project phase an activity called "Start Phase X" where "X" refers to the phase of work. The "Start Phase X" activity shall have an "Early Start" constraint date equal to the date on which the NTP was acknowledged, and a zero day duration.
d. End Phase: The Contractor shall include as the last activity in a project phase an activity called "End Phase X" where "X" refers to the phase of work. The "End Phase X" activity shall have a "Late Finish" constraint date equal to the completion date for the project, and a zero day duration.
e. Phase X: The Contractor shall include a hammock type activity for each project phase called "Phase X" where "X" refers to the phase of work. The "Phase X" activity shall be logically tied to the earliest and latest activities in the phase.
4. Default Progress Data Disallowed: Actual Start and Finish dates shall not be automatically updated by default mechanisms that may be included in CPM scheduling software systems. Actual Start and Finish dates on the CPM schedule shall match those dates provided from Contractor Quality Control Reports. Failure of the Contractor to document the Actual Start and Finish dates on the Daily Quality Control report for every in-progress or completed activity, and failure to ensure that the data contained on the Daily Quality Control reports is the sole basis for schedule updating shall result in the disapproval of the Contractor's schedule and the inability of the Contracting Officer to evaluate Contractor progress for payment purposes. Updating of the percent complete and the remaining duration of any activity shall be independent functions. Program features which calculate one of these parameters from the other shall be disabled.

5. Out-of-Sequence Progress: Activities that have posted progress without all preceding logic being satisfied (Out-of-Sequence Progress) will be allowed only on a case-by-case approval of the Contracting Officer. The Contractor shall propose logic corrections to eliminate all out of sequence progress or justify not changing the sequencing for approval prior to submitting an updated project schedule.
6. Negative Lags: Lag durations contained in the project schedule shall not have a negative value.
D. Malabang Trail Project Schedule Submissions: The Contractor shall provide the submissions as described below. The data reports and network diagrams required for each submission are contained in paragraph 4.3 SUBMITTAL PROCEDURES.
E. The Contracting Officer will approve activity progress, proposed revisions, and adjustments as appropriate. AF Form 3065 Progress Report is used for this activity.

SCHEDULE OF VALUES
A. Description of Work

1. This specification covers the preparation, content and submittal of the schedule of values.
The Schedule of Values is an itemized list that establishes the value or cost of each part of the Work. It shall be used as the basis for preparing progress payments.
B. Preparation
1. Schedule shall show breakdown of labor, materials equipment and other costs as directed by the Contracting Officer.
2. Costs shall be in sufficient detail to indicate separate amounts for each major subsection of the Work. The Contractor may include an item for bond, insurance, temporary facilities and job mobilization.
3. Schedule of Values shall be prepared on 8-1/2-inch by 11-inch white paper.
4. Use the major subsections of the Detailed Scope of Work as the basis for Schedule format. List sub-items of major products or systems as appropriate or when requested by the Contracting Officer.
5. When requested by the Contracting Officer, support values with data that will substantiate their correctness.
6. The sum of the individual values shown on the Schedule of Values must equal the total Contract Price.
7. Schedule shall show the purchase and delivery costs for materials and equipment that the Contractor anticipates he shall request payment for prior to their installation.
C. Submittal: Submit two copies of Schedule, or any other number of copies as directed by the Contracting Officer, to the Contracting Officer for approval at least 20 days prior to submitting first application for a progress payment. After review by the Contracting Officer, revise and resubmit Schedule as required until it is approved.

B. Submittals

C. Regulatory Requirements
1. In addition to the detailed requirements included in the provisions of this contract, work performed shall comply with USACE EM 385-1-1, dated 30 November 2014, and the State of Texas laws, ordinances, criteria, rules and regulations. Submit matters of interpretation of standards to the appropriate administrative agency for resolution before starting work. Where the requirements of this specification, applicable laws, criteria, ordinances, regulations, and referenced documents vary, the most stringent requirements shall apply.
D. Site Qualifications, Duties and Meetings
1. Personnel Qualifications
a. Site Safety and Health Officer (SSHO): Site Safety and Health Officer (SSHO) shall be provided at the work site at all times to perform safety and occupational health management, surveillance, inspections, and safety enforcement for the Contractor.
The Contractor Quality Control (QC) person can be the SSHO on this project. The SSHO shall meet the following requirements:
1) Level 2: A minimum of 3 years safety work on similar project. 30-hour OSHA construction safety class or equivalent within last 3 years.

2. Preconstruction Conference: Contractor representatives who have a responsibility or significant role in accident prevention on the project shall attend the preconstruction conference. This includes the project superintendent, site safety and health officer, quality control supervisor, or any other assigned safety and health professionals who participated in the development of the APP (including the Activity Hazard Analyses (AHAs) and special plans, program and procedures associated with it).
a. The Contractor shall discuss the details of the submitted APP to include incorporated plans, programs, procedures and a listing of anticipated AHAs that will be developed and implemented during the performance of the contract. This list of proposed AHAs will be reviewed at the conference and an agreement will be reached between the Contractor and the Contracting Officer's representative as to which phases will require an analysis. In addition, a schedule for the preparation, submittal, review, and acceptance of AHAs shall be established to preclude project delays.
b. Deficiencies in the submitted APP will be brought to the attention of the Contractor at the preconstruction conference, and the Contractor shall revise the plan to correct deficiencies and re-submit it for acceptance. Work shall not begin until there is an accepted APP.
c. Safety Meetings: Shall be conducted and documented as required by EM 385-1-1.
Minutes showing contract title, signatures of attendees and a list of topics discussed shall be attached to the Contractors' daily quality control report.
E. Accident Prevention Plan (APP)
1. The Contractor shall use a qualified person to prepare the written site-specific APP.
Prepare the APP in accordance with the format and requirements of USACE EM 385-1-1 and as supplemented herein. Cover all paragraph and subparagraph elements in USACE EM 385-1-1, Appendix A, "Minimum Basic Outline for Accident Prevention Plan".
Specific requirements for some of the APP elements are described below. The APP shall be job-specific and shall address any unusual or unique aspects of the project or activity for which it is written. The APP shall interface with the Contractor's overall safety and health program. Any portions of the Contractor's overall safety and health program referenced in the APP shall be included in the applicable APP element and made site-specific. The Government considers the Prime Contractor to be the "controlling authority" for all work site safety and health of the subcontractors. Contractors are responsible for informing their subcontractors of the safety provisions under the terms of the contract and the penalties for noncompliance, coordinating the work to prevent one craft from interfering with or creating hazardous working conditions for other crafts, and inspecting subcontractor operations to ensure that accident prevention responsibilities are being carried out. The APP shall be signed by the person and firm preparing the APP, the Contractor, the on-site superintendent, the designated site safety and health officer.
Submit the APP to the Contracting Officer 7 calendar days prior to the date of starting work. Work cannot proceed without an accepted APP. Once accepted by the Contracting Officer, the APP and attachments will be enforced as part of the contract. Disregarding the provisions of this contract or the accepted APP will be cause for stopping of work, at the discretion of the Contracting Officer, until the matter has been rectified. Once work begins, changes to the accepted APP shall be made with the knowledge and concurrence of the Contracting Officer, project superintendent, SSHO and quality control manager.

Should any hazard become evident, stop work in the area, secure the area, and develop a plan to remove the hazard. Notify the Contracting Officer within 24 hours of discovery.
Eliminate/remove the hazard. In the interim, all necessary action shall be taken to restore and maintain safe working conditions in order to safeguard onsite personnel, visitors, the public (as defined by ANSI/ASSE A10.34,) and the environment. Copies of the accepted plan will be maintained at the job site. The APP shall be continuously reviewed and amended, as necessary, throughout the life of the contract. Unusual or high-hazard activities not identified in the original APP shall be incorporated in the plan as they are discovered.
2. EM 385-1-1 Contents: In addition to the requirements outlines in Appendix A of USACE EM 385-1-1, the following is required:
a. Names and qualifications of competent and of qualified persons. As a minimum, competent persons shall be designated and qualifications submitted for each of the following major areas: scaffolding; fall protection; hazardous energy; confined space;
health hazard recognition, evaluation and control of chemical, physical and biological agents; personal protective equipment and clothing to include selection, use and maintenance.
b. Crane Critical Lift Plan. Prepare and sign weight handling critical lift plans for lifts of personnel; and lifts involving non-routine rigging or operation, sensitive equipment, or unusual safety risks. The plan shall be submitted 15 calendar days prior to on-site work and include the requirements of USACE EM 385-1-1, paragraph 16.C.18. and the following:
1) For lifts of personnel, the plan shall demonstrate compliance with the requirements of 29 CFR 1926.550(g).
3. Fall Protection and Prevention (FP&P) Plan: The plan shall be site specific and address all fall hazards in the work place and during different phases of construction. It shall address how to protect and prevent workers from falling to lower levels when they are exposed to fall hazards above 1.8 m (6 feet). A qualified person for fall protection shall prepare and sign the plan. The plan shall include fall protection and prevention systems, equipment and methods employed for every phase of work, responsibilities, assisted rescue, self-rescue and evacuation procedures, training requirements, and monitoring methods. Fall Protection and Prevention Plan shall be revised every six months for lengthy projects, reflecting any changes during the course of construction due to changes in personnel, equipment, systems or work habits. The accepted Fall Protection and Prevention Plan shall be kept and maintained at the job site for the duration of the project.
The Fall Protection and Prevention Plan shall be included in the Accident Prevention Plan (APP).
4. Occupant Protection Plan: Provide a safety and health plan with respects to lead-based paint removal, asbestos abatement, demolition and new roof repairs or replacement.
F. Activity Hazard Analysis (AHA)
1. The Activity Hazard Analysis (AHA) format shall be in accordance with USACE EM 385-1-1. Submit the AHA for review at least 7 calendar days prior to the start of each phase of work. Format subsequent AHAs as amendments to the APP. The analysis should be used during daily inspections to ensure the implementation and effectiveness of the

The AHA list will be reviewed periodically at least monthly at the Contractor supervisory safety meeting and updated as necessary when procedures, scheduling, or hazards change. The activity hazard analyses shall be developed using the project schedule as the basis for the activities performed. Any activities listed on the project schedule will require an AHA. The AHAs will be developed by the contractor, supplier or subcontractor and provided to the prime Contractor for submittal to the Contracting Officer.
G. REPORTS
1. Accident Reports: For recordable injuries and illnesses, and property damage accidents resulting in at least $2,000 in damages, the Prime Contractor shall conduct an accident investigation to establish the root cause(s) of the accident, complete the USAF Mishap Report Form AF IMT 711B and provide the report to the Contracting Officer within five
(5) calendar days of the accident. The Contracting Officer will provide copies of any required or special forms.
2. Accident Notification: Notify the Contracting Officer as soon as practical, but not later than 24 hours after any accident meeting the definition of Recordable Injuries or Illnesses or High Visibility Accidents, property damage equal to or greater than $2,000, or any accident that causes major injury or death. Information shall include contractor name;
contract title; type of contract; name of activity, installation or location where accident occurred; date and time of accident; names of personnel injured; extent of property damage, if any; extent of injury, if known, and brief description of accident (to include type of construction equipment used, PPE used, etc.). Preserve the conditions and evidence on the accident site until the Government investigation team arrives on-site and Government investigation is conducted.
H. HOT WORK
1. Prior to performing "Hot Work" (welding, cutting, etc.) or operating other flame-producing/spark producing devices, a written permit shall be requested from the Fort Sam Houston Fire Department. CONTRACTORS ARE REQUIRED TO MEET ALL CRITERIA BEFORE A PERMIT IS ISSUED. The Contractor will provide at least two (2)-twenty (20) pound 4A:20 BC rated extinguishers for normal "Hot Work". All extinguishers shall be current inspection tagged, approved safety pin and tamper resistant seal. It is also mandatory to have a designated FIRE WATCH for any "Hot Work" done at this activity. The Fire Watch shall be trained in accordance with NFPA 51B and remain on-site for a minimum of thirty (30) minutes after completion of the task or as specified on the hot work permit. When starting work in the facility, Contractors shall require their personnel to familiarize themselves with the location of the nearest fire alarm boxes and place in memory the emergency Fire Department phone number. ANY
FIRE, NO MATTER HOW SMALL, SHALL BE REPORTED TO THE
RESPONSIBLE FIRE DEPARTMENT IMMEDIATELY.
I. Fall Protection Anchorage
1. Fall protection anchorage, conforming to ANSI Z359.1, installed under the supervision of a qualified person in fall protection, shall be left in place for continued customer use and
Rev: 07 May 2018 Page 16 of 75 so identified by signage stating the capacity of the anchorage (strength and number of persons who may be tied-off to it at any one time).
1.2 PRODUCTS – NOT APPLICABLE
A. Construction and/or Other Work
1. The Contractor shall comply with USACE EM 385-1-1, NFPA 241, the APP, the AHA, Federal and/or State OSHA regulations, and other related submittals and activity fire and safety regulations. The most stringent standard shall prevail.
B. Fall Hazard Protection and Prevention Program
1. The Contractor shall establish a fall protection and prevention program, for the protection of all employees exposed to fall hazards. The program shall include company policy, identify responsibilities, education and training requirements, fall hazard identification, prevention and control measures, inspection, storage, care and maintenance of fall protection equipment and rescue and evacuation procedures.
2. Training: The Contractor shall institute a fall protection training program. As part of the Fall Hazard Protection and Prevention Program, the Contractor shall provide training for each employee who might be exposed to fall hazards. A competent person for fall protection shall provide the training. Training requirements shall be in accordance with USACE EM 385-1-1, section 21.A.16.
3. Fall Protection Equipment and Systems: The Contractor shall enforce use of the fall protection equipment and systems designated for each specific work activity in the Fall Protection and Prevention Plan and/or AHA at all times when an employee is exposed to a fall hazard. Employees shall be protected from fall hazards as specified in EM 385-1-1.
Personal fall arrest systems are required when working from an articulating or extendible boom, swing stages, or suspended platform. In addition, personal fall arrest systems are required when operating other equipment such as scissor lifts if the work platform is capable of being positioned outside the wheelbase. The need for tying-off in such equipment is to prevent ejection of the employee from the equipment during raising, lowering, or travel. Fall protection must comply with 29 CFR 1926.500, Subpart M, USACE EM 385-1-1 and ANSI A10.32.
a. Personal Fall Arrest Equipment: Personal fall arrest equipment, systems, subsystems, and components shall meet ANSI Z359.1. Only a full-body harness with a shock-absorbing lanyard or self-retracting lanyard is an acceptable personal fall arrest body support device. Body belts may only be used as a positioning device system (for uses such as steel reinforcing assembly and in addition to an approved fall arrest system).
Harnesses shall have a fall arrest attachment affixed to the body support (usually a Dorsal D-ring) and specifically designated for attachment to the rest of the system.
Only locking snap hooks and carabiners shall be used. Webbing, straps, and ropes shall be made of synthetic fiber. The maximum free fall distance when using fall arrest equipment shall not exceed 1.8 m (6 feet). The total fall distance and any swinging of the worker (pendulum-like motion) that can occur during a fall shall always be taken into consideration when attaching a person to a fall arrest system.

4. Fall Protection for Roofing Work: Fall protection controls shall be implemented based on the type of roof being constructed and work being performed. The roof area to be accessed shall be evaluated for its structural integrity including weight-bearing capabilities for the projected loading.
a. Low Sloped Roofs:
1) For work within 1.8 m (6 feet) of an edge, on low-slope roofs, personnel shall be protected from falling by use of personal fall arrest systems, guardrails, or safety nets.
2) For work greater than 1.8 m (6 feet) from an edge, warning lines shall be erected and installed in accordance with 29 CFR 1926.500 and USACE EM 385-1-1.
b. Steep-Sloped Roofs: Work on steep-sloped roofs requires a personal fall arrest system, guardrails with toe-boards, or safety nets. This requirement also includes residential or housing type construction.
5. Existing Anchorage: Existing anchorages, to be used for attachment of personal fall arrest equipment, shall be certified (or re-certified) by a qualified person for fall protection in accordance with ANSI Z359.1. Exiting horizontal lifeline anchorages shall be certified (or re-certified) by a registered professional engineer with experience in designing horizontal lifeline systems.
6. Horizontal Lifelines: Horizontal lifelines shall be designed, installed, certified and used under the supervision of a qualified person for fall protection as part of a complete fall arrest system which maintains a safety factor of 2 (29 CFR 1926.500).
7. Guardrails and Safety Nets: Guardrails and safety nets shall be designed, installed and used in accordance with EM 385-1-1 and 29 CFR 1926 Subpart M.
8. Rescue and Evacuation Procedures When personal fall arrest systems are used, the contractor must ensure that the mishap victim can self-rescue or can be rescued promptly should a fall occur. A Rescue and Evacuation Plan shall be prepared by the contractor and include a detailed discussion of the following: methods of rescue; methods of self-rescue; equipment used; training requirement; specialized training for the rescuers;
procedures for requesting rescue and medical assistance; and transportation routes to a medical facility. The Rescue and Evacuation Plan shall be included in the Activity Hazard Analysis (AHA) for the phase of work, in the Fall Protection and Prevention (FP&P) Plan, and the Accident Prevention Plan (APP).
C. Scaffolding
1. Employees shall be provided with a safe means of access to the work area on the scaffold. Climbing of any scaffold braces or supports not specifically designed for access is prohibited. Access to scaffold platforms greater than 6 m (20 feet) in height shall be accessed by use of a scaffold stair system. Vertical ladders commonly provided by scaffold system manufacturers shall not be used for accessing scaffold platforms greater than 6 m (20 feet) in height. The use of an adequate gate is required. Contractor shall ensure that employees are qualified to perform scaffold erection and dismantling. Do not use scaffold without the capability of supporting at least four times the maximum intended load or without appropriate fall protection as delineated in the accepted fall protection and prevention plan. Stationary scaffolds must be attached to structural building components to safeguard against tipping forward or backward. Special care shall be given to ensure scaffold systems are not overloaded. Side brackets used to extend
Rev: 07 May 2018 Page 18 of 75 scaffold platforms on self-supported scaffold systems for the storage of material is prohibited. The first tie-in shall be at the height equal to 4 times the width of the smallest dimension of the scaffold base. Work platforms shall be placed on mud sills. Scaffold or work platform erectors shall have fall protection during the erection and dismantling of scaffolding or work platforms that are more than six feet. Delineate fall protection requirements when working above six feet or above dangerous operations in the Fall Protection and Prevention (FP&P) Plan and Activity Hazard Analysis (AHA) for the phase of work.
2. Stilts: The use of stilts for gaining additional height in construction, renovation, repair or maintenance work is prohibited.
D. Equipment
1. Material Handling Equipment: Material handling equipment such as forklifts shall not be modified with work platform attachments for supporting employees unless specifically delineated in the manufacturer's printed operating instructions.
2. The use of hooks on equipment for lifting of material must be in accordance with manufacturer's printed instructions.
3. Operators of forklifts or power industrial trucks shall be licensed in accordance with
OSHA.
E. ELECTRICAL
1. Portable Extension Cords: Portable extension cords shall be sized in accordance with manufacturer ratings for the tool to be powered and protected from damage. All damaged extension cords shall be immediately removed from service. Portable extension cords shall meet the requirements of NFPA 70.

B. Description of Work
1. This section provides additional requirements for implementing the accident prevention provisions of EM 385-1-1, and specifies a Site Safety and Health Plan (SSHP) which shall satisfy the requirements for submission of a separate Accident Prevention Plan (APP) as required by EM 385-1-1.
1. Government approval is required for submittals with a "GA" designation; submittals having an "FIO" designation are for information only. The following shall be submitted in accordance with Section 01 30 00 SUBMITTAL PROCEDURES:

Work performed under this Contract shall comply with EM 385-1-1, applicable Federal, state, and local safety and occupational health laws and regulations. This includes, but is not limited to, Rev: 07 May 2018 Page 20 of 75

D. Preconstruction Safety Conference
1. Prior to construction, meet with the Contracting Officer to discuss and to review safety procedures.
E. Safety and Health Program
1. Integrate the site-specific program requirements of the OSHA Standards into one site-specific document, the Site Safety and Health Plan (SSHP). The SSHP shall interface with the employer's overall Safety and Health Program. Any portions of the overall Safety and Health Program that are referenced in the SSHP shall be included as appendices to the SSHP.
F. Site Safety and Health Plan
1. Preparation and Implementation: Prepare a Site Safety and Health Plan (SSHP) covering onsite work to be performed by the Contractor and all subcontractors. The SSHP shall establish, in detail, the protocols necessary for the anticipation, recognition, evaluation, and control of hazards associated with each task performed. The SSHP shall address site-specific safety and health requirements and procedures based upon site-specific conditions. The level of detail provided in the SSHP shall be tailored to the type of work, complexity of operations to be performed, and hazards anticipated. Details about some activities may not be available when the initial SSHP is prepared and submitted.
Therefore, the SSHP shall address, in as much detail as possible, anticipated tasks, their related hazards and anticipated control measures. Additional details shall be included in the activity hazard analyses as described in paragraph ACTIVITY HAZARD
ANALYSES.
2. Acceptance and Modifications: Prior to submittal, the SSHP shall be signed and dated by Site Superintendent. The SSHP shall be submitted for review fourteen (14) days prior to the start of construction. Deficiencies in the SSHP shall be identified by the Contracting Officer and returned. The SSHP shall be revised to correct the deficiencies and resubmitted for acceptance. Onsite work shall not begin until the plan has been accepted.
A copy of the written SSHP shall be maintained onsite. As work proceeds, the SSHP shall be adapted to new situations and new conditions. Changes and modifications to the accepted SSHP shall be made with the knowledge and concurrence of the Site Superintendent, and the Contracting Officer. Should any unforeseen hazard become evident during the performance of the work, the Site Superintendent shall bring such hazard to the attention of the Contracting Officer, both verbally and in writing, for resolution as soon as possible. In the interim, necessary action shall be taken to re-establish and maintain safe working conditions in order to safeguard onsite personnel, visitors, the public, and the environment. Disregard for the provisions of this specification or the accepted SSHP shall be cause for stopping of work until the matter has been rectified.
3. Availability: The SSHP shall be made available in accordance with 29 CFR 1910, Section .120 (b) (1) (v) and 29 CFR 1926, Section .65 (b) (1) (v).

.65 (b) (4) and the Accident Prevention Plan as described in Appendix A of EM 385- 1-1 and those described in this section shall be addressed in the SSHP. Where the use of a specific topic is not applicable to the project, the SSHP shall include a statement to justify its omission or reduced level of detail and establish that adequate consideration was given the topic.

c. Unexploded Ordnance (UXO): Unexploded ordnance (aerial practice bombs, mortar bombs, artillery shells, grenades, rifle cartridges, etc.) may be encountered at some sites on Fort Sam Houston and almost any work site at Camp Bullis. The contractor will be notified whether or not unexploded ordnance has been discovered in the past in the work sites, borrow areas, haul routes, etc. The Natural Resources Offices at Fort Sam Houston and Camp Bullis maintain current information on unexploded ordnance.
H. Plan Requirements
1. The SSHP shall include a site description and contamination characterization section that addresses the following elements:
a. Description of site location, topography, size and past uses of the site.
b. A list of contaminants that may present occupational health and safety hazards. This list shall be created by evaluating the analytical results in this section and by researching sources of information from past site investigation activities. Chemical names, concentration ranges, media in which found, locations onsite, and estimated quantities/volumes to be impacted by site work shall be included if known. The contamination characterization shall be reviewed and revised if new chemicals are identified as work progresses.
I. Unexploded Ordinance (UXO)
1. The contractor shall be notified of the likelihood of encountering unexploded ordnance in the work site, borrow areas, haul routes, etc.
