STATEMENT OF WORK

Project #CYRB110023; Repair Malabang Trail

JBSA – Camp Bullis

1. PROJECT SCOPE

1.1 BACKGROUND

The purpose of this project is the replacement of the pavement on Malabang Trail, beginning at the at the intersection of Malabang Trail and Sewell Rd and ending at the intersection of Malabang Trail and County Rd, a distance of approximately 25,355 feet, on Joint Base San Antonio – Camp Bullis, San Antonio, Texas (see SOW Attachment 1).

1.2 GENERAL

1.2.1 The pavement on Malabang Trail is damaged by heavy vehicles and is at the end of its lifecycle and needs to be replaced.

1.3 CONSTRUCTION

Asphalt Pavement-The government requires existing asphalt pavement be milled and overlaid with new asphalt. New asphalt pavement shall match existing pavement width. 2 foot wide shoulders made from A2 base shall be constructed on each side of the road with a minimum 3-inch compacted base thickness to match the height of the new pavement. The pavement shall be 3-inches thickness of Type D HMAC, TxDOT Item 340. When the base course is approved, the first 1-1/2 inch lift of Type D HMAC will be laid and compacted directly onto the base course without prime coat. When the construction of the first lift of Type D is approved, the Contractor shall apply tack coat at the rate of 0.1 gallons per square yard and then construct the final lift of 1-1/2 Type D HMAC.

1.3.1 Construction Surveying-Contractor shall provide surveying or survey related activity necessary to provide pavement horizontal and vertical location, control grade, thickness, and smoothness of the base and the pavement.

1.3.2 Traffic Control Plan-The Contractor shall prepare and submit a Traffic Control Plan drawn in AutoCAD. The Traffic Control Plan shall provide alternate access to all buildings on Malabang Trail impacted by road closures and detours required for construction. The Traffic Control Plan shall clearly show all detour routes, detour signs, and barricade locations. The Traffic Control plan shall be prepared in accordance with the Texas Manual of Uniform traffic Control Devices (MUTCD). The Contractor shall deliver the Traffic Control Plan 30 days prior to start of construction to provide the Government Project Manager time to prepare and process the electronic Staff Summary Sheet (eSSS) required to get road closures and detours approved by the Air Base Wing Commander. The Contractor will be responsible for setting up and maintaining all traffic control devices during the entire period of construction and removal when no longer needed.

1.3.3 Storm Water Pollution Prevention Plan. Shall include NPDES Permit and construction of: Silt Fence, Inlet Protection, Concrete Washout Pit, and any other Best Management Practices required by laws and regulations.

1.3.4 Striping Plan- Contractor shall restripe the new pavement with 120 mils thickness reflective thermoplastic paint for the edge lines, centerlines, stop bars, painted islands and arrows. Contractor shall raise (example with a sleeve) any utility access covers or man holes, as needed.

1.3.5 Testing of asphalt, base, and subgrade shall be per specifications. Contractor’s testing subcontractor shall take temperature readings of the asphalt as soon as it arrives on site and during placement of asphalt and record the temperature readings and immediately inform Contractor’s QA and Govt. Representative if readings do not meet specifications.

1.3.6 Potholing -Contractor is responsible for patching pavement from potholing if done prior to road closure.

1.3.7 Restoration of Turf is required for all areas disturbed by the Contractor outside of the road and shoulder areas by sodding or hydromulch including construction of a temporary irrigation system to water turf until it is restored or use a water tanker truck to water turf until it is restored.

1.3.8 Pavement Removal- pavement removal by milling is preferred method.

1.3.9 Reworking Base-The Contractor shall rework the existing base by bringing in new base material as required to meet proposed grade and 2% grade cross slope and rolling it and moisturizing it until it achieves COSA compaction requirements.

1.4 Demolition

1.4.1 Contractor shall protect government equipment and property from damage during construction. Contractor is responsible for properly documenting the condition of the property prior to the start of construction. Contractor shall be responsible for repairing any damages to government property due to contractor’s negligence, at contractor’s own cost.

1.4.2 At the conclusion of the project, all demolished materials will become the property of the contractor and shall be removed off-site and disposed in accordance with all applicable local, state, and federal laws.

1.5 CODES AND REGULATIONS

All work shall be accomplished in accordance with the latest edition of applicable codes, standards, regulations and other requirements. In the case where two or more standards are applicable, the contractor shall ensure compliance with the more stringent of the standards as determined by the 502 CES PM.

2 GENERAL

2.1 GENERAL REQUIRMENTS

2.1.1 Site Plan - The contractor shall prepare a site plan indicating the proposed location and dimensions of any area to be isolated/fenced for use by the contractor, avenues of ingress/egress to the project area, and laydown/storage area. Any areas, which may have to be graveled to prevent the tracking of mud, shall also be identified. The contractor shall also indicate if the use of a supplemental or other staging area is desired. Contractor site plan shall be submitted for approval by the government. Contractor shall coordinate and obtain approval from the 502nd CE Project Manager for location of laydown yard.

2.1.2 Identification of Employees - The contractor shall be responsible for furnishing to each employee, and for requiring each employee engaged on the work to display, identification as approved and directed by the Contracting Officer. Prescribed identification shall immediately be delivered to the Contracting Officer for cancellation upon release of any employee. All contractor and sub- contractor personnel shall wear identifying markings on hard hats and safety vests clearly identifying the company for whom the employee works.

2.1.3 Employee Parking - Contractor employees shall park privately owned vehicles in an area designated by the Contractor to the 502d CE/PM and Contracting Officer. This area will be within reasonable walking distance of the construction site. Contractor employee parking shall not interfere with existing and established parking requirements of the military installation. Contractor shall provide the 502nd CE Project Manager the location and map.

2.1.4 Job Site Tools and Equipment - All hand tools and equipment shall be maintained in good working conditions and properly stored when not in use.

Construction attire shall be IAW applicable OSHA regulations. All tools and equipment shall be at hand prior to the contractor commencing work for the day.

2.2 AVAILABILITY AND USE OF UTILITY SERVICES

2.2.1 All temporary utilities are the responsibility of the Contractor to provide and fund.

The contractor, at its expense and in a manner satisfactory to the Contracting Officer, shall provide and maintain necessary temporary connections and distribution lines.

2.2.2 In order to hook up to the fire hydrant on JBSA the contractor shall provide a meter and a backflow preventer. The backflow prevention device shall be approved prior to installation by JBSA Backflow Program Manager via Project Manager (PM)/ Inspector.

In order to test a backflow preventer the contractor must have a licensed backflow prevention assembly tester. A copy of their license shall be provided. The test gauge used by the tester must be tested annually for accuracy and a copy of the last test report must be provided. All backflow prevention devices shall be tested upon installation and annually thereafter until removal. A copy of the Testing and Maintenance (T&M) report shall be provided. Ensure that the meter (where applicable) and the backflow are the only things connected to the fire hydrant. Any other hoses, fittings, pipes etc. cannot be connected until after the device has passed inspection. The PM/ Inspector will witness the test and ensure the Fire Dept. is notified of the hydrant being used. The backflow device cannot be the open/close (isolation) valve for the water flow. The hydrant nut must be operated with an approved spanner wrench (no pipe wrenches) when water is needed. Hydrants are not to remain in the “on” position. At the completion of the project or when the hydrant is no longer needed, the contractor shall coordinate with PM/ Inspector to ensure Fire Dept. is aware that the hydrant is no longer in use.

2.2.3 If the use of another backflow device at another job site or another location on your current job site is required, the process shall be repeated. Every time a new connection is made to the potable system backflow device must be tested.

2.2.4 Hooking up a backflow device to a non-potable source (i.e. sprinkler system)

SOW- #CYRB110023-Malabang Trail Page Page 4 of 12 requires the licensed tester to use a dedicated non-potable test gauge and Backflow must be marked “Non Potable Use Only”. Once a backflow device has been used within a non- potable system, it can never be returned to us on a potable water system.

2.2.5 Electricity will not be provided by the government.

2.2.6 Sanitation - The contractor shall be responsible to provide and maintain within the construction area minimum field-type sanitary facilities. Government toilet facilities will not be available to contractor’s personnel. Contractor shall bring a sanitary station (Port- O-John) to have on site or at laydown yard. Sanitary station shall be secured such that high winds will not push it over.

2.2.7 Telephone - The contractor shall make all necessary arrangements and pay all costs for telephone facilities if desired.

2.3 CONTRACTOR'S TEMPORARY FACILITIES

2.3.1 Storage Area - The government will provide a laydown yard. The laydown yard shall be returned to the original condition and grass shall be restored prior to final payment. A submittal for the laydown yard is required. Submittal shall include plan of the laydown yard, number of facilities inside the laydown yard, and haul route. Allow about 3 weeks for approval. Contractor may not mobilize until laydown yard is approved.

The contractor shall construct and maintain temporary 6-foot high chain link fence around the exterior of the trailers and stored materials. The fence shall include woven privacy fabric, bound edging with brass 1”-grommets fastening locations; factory cut wind-holes; color: dark brown, so that visibility through the fence is obstructed. Fence fabric shall be installed on the “public” side of the site’s construction fence and maintained in a neat and tight fashion onto the fence.

Fence fabric damaged during the construction phase shall be replaced at the direction of the Contracting Officer. Fence fabric material color shall be dark brown and submitted to the Contracting Officer for approval. Fence posts may be driven, in lieu of concrete bases, where soil conditions permit. Materials, or equipment shall not be placed or stored outside the fenced area unless such materials, or equipment are assigned a separate and distinct storage area by the Contracting Officer away from the vicinity of the construction site but within the military boundaries. Trailers, equipment, or materials shall not be open to public view with the exception of those items which are in support of ongoing work on any given day.

Materials shall not be stockpiled outside the fence in preparation for the next day's work.

Mobile equipment, such as tractors, wheeled lifting equipment, cranes, trucks, and like equipment, shall be parked within the fenced area at the end of each workday.

2.3.2 Maintenance of Storage Area/Laydown Yard - Fencing shall be kept in a state of good repair and proper alignment. Should the contractor elect to traverse with construction equipment or other vehicles, grassed or unpaved areas, which are not established roadways, such areas, shall be covered with a layer of gravel as necessary to prevent rutting and the tracking of mud onto paved or established roadways; gravel gradation shall be at the contractor's discretion. The contractor shall mow the grass located within the boundaries of the construction site and the contractor shall mow five

(5) feet outside the boundaries of the construction site for the duration of the project.

Grass and vegetation along fences, buildings, under trailers, and in areas not accessible to mowers shall be edged or trimmed neatly. Contractor shall remove debris from the fence line daily.

2.3.3 Security Provisions - Adequate outside security lighting shall be provided at the contractor's laydown yard, stockpile and during night construction. The contractor shall be responsible for the security of contractor’s own equipment; in addition, the contractor shall notify appropriate law enforcement agency requesting periodic security checks of the temporary project field office.

2.3.4 Removal of Temporary Fence - The contractor shall remove the temporary fence upon completion and acceptance of the project. In case the fence needs to be removed for the completion of obstructed areas, such as landscaping, the site shall remain in neat appearance with all proper barricades, as specified.

2.4 BULLETIN BOARD, PROJECT SIGN, AND PROJECT SAFETY SIGN

2.4.1 Bulletin Board - Immediately upon beginning of work, the contractor shall displaying the Equal Employment Opportunity poster, a copy of the wage decision contained in the contract, Wage Rate Information poster, and other information approved by the Contracting Officer. All information shall be located at the project site in a conspicuous place easily accessible to all employees, as approved by the Contracting Officer. Legible copies of the aforementioned data shall be displayed until work is completed. Upon completion of work the binder shall be removed by and remain the property of the contractor.

2.4.2 Project and Safety Signs - The contractor shall prominently display any safety signs necessary to prevent any accidents to contractor’s employees, government employees, or any other visitors in the project area. The signs shall be posted within 10 working days after receipt of the notice to proceed (NTP) for construction. The signs posted shall help anyone understand they aren’t allowed in construction area and allowed an alternate route to their location. Upon completion of the project, the signs shall be removed from the site.

2.5 CLEANUP

At the end of each working day, contractor shall clean up the work site by the removal of all construction debris, waste materials, packaging material and the like. If the aforementioned items could become airborne, they shall be placed in secure refuse containers immediately. Clean up of the work site shall include the construction areas, material storage areas, parking and eating areas, and any other areas affected by the construction process. Any dirt or mud, tracked on to any street, parking area, sidewalk, patio, driveway, or building shall be cleaned away daily. Materials resulting from demolition activities that are salvageable shall be stored within the laydown area described above or at the supplemental storage area. All stored material, whether new or salvaged, shall be neatly stacked when stored and protected from weather damage.

2.6 RESTORATION OF GOVERNMENT AREA

Areas used by the contractor for the storage of equipment or material, or other use, shall be restored to the original or better condition prior to opening to the public or returning to the government.

2.7 ENVIRONMENTAL MANAGEMENT

The contractor shall comply with all provisions of JBSA Environmental Protection specification as applicable to this contract. Appendix A includes a copy of the JBSA Environmental Protection specification. No separate payment will be made to the contractor to ensure compliance with the provisions of the environmental specifications; all cost shall be part of the contractor’s base bid.

3 EXISTING UTILITIES

Any utility line made known to the Contractor and damaged during construction work shall be repaired immediately by the contractor at no cost to the government. It shall be the responsibility of the contractor to protect the utility markings made by the government during the issuance of Digging Permit throughout the duration of the contract period.

4 SUBMITTALS

The contractor shall update the provided AF Submittal Registry with additional items for government approval to comply with the intent of this SOW. These items shall include all newly installed products and/or reference material that are to remain government property at the conclusion of this project.

Contractor shall utilize the updated Submittal Registry to provide submittals on AF IMT 3000 Material Approval Submittal Form for all listed items, prior to work or installation of materials. Do not purchase or conduct work until the approved submittal form has been returned from 502 CONS.

The contractor shall allow up to 14 days for government review of all submittals for approval or disapproval.

5 PRE-CONSTRUCTION MEETING

The contractor shall be required to attend a preconstruction meeting with the Contracting Officer prior to the commencement of any construction work. The preconstruction meeting shall be attended by at minimum the contractor’s Project Manager, Site Superintendent, Quality Control, and Site Safety and Health Officer.

6 QUALITY ASSURANCE

6.1 Provide a Quality Control (QC) person at the work site at all times for continuous surveillance of project activities to ensure workmanship, methods, materials, and equipment conform to contract requirements. QC shall have the “stop work” authority to prevent or stop performance inconsistent with the contract requirements.

6.2 The contractor shall submit a Quality Control Plan (QCP) for government approval to the Contracting Officer at least 14 calendar days prior to commencing work. Work cannot proceed without an approved QCP. At minimum, the QCP shall discuss the following topics:

7 GENERAL SAFETY REQUIREMENTS

7.1 Provide a Site Safety and Health Officer (SSHO) at the work site at all times to perform safety and occupational health management, surveillance, inspections, and safety enforcement for the Contractor. The Contractor QC can be SSHO on this project.

Contractor shall use and comply with EM385-1-1 Safety Manual dated 2014 also known as the Red book (online in PDF).

7.2 The contractor shall submit a site specific Accident Prevention Plan (APP) for government approval to the Contracting Officer at least 14 calendar days prior to commencing work.

Work cannot proceed without an approved APP. All changes to the approved APP shall be made with the knowledge and concurrence of the Contracting Officer and 502 CES PM. At minimum, the APP shall follow the EM385-1-1abbreviated safety plan.

7.3 Obey the posted base speed limits. Wear Seat belts on base IAW AFI 91-207.

7.4 Report contractor accidents involving injury to Air Force personnel or damage to government property to the Contracting Officer, 502 CES PM, 502 CES Inspector and the 502 ABW OL- A/SEG, Ground Safety Officer immediately.

7.5 The Contractor shall familiarize and comply with the general safety requirements IAW DOL regulations, OSHA 29 CFR Part 1926, as well as applicable AFOSH standards and AFRs. These standards and AFRs are available in the 502 ABW OL-A/SEG, Ground Safety Office, building 2484.

8 BARRICADES AND WARNING SIGNS

Adequate barricades, warning signs and flares shall be furnished, installed, and maintained by the contractor. Barricades and signs shall meet the requirements established and in the National Manual on Uniform Traffic Control Devices for Streets and Highways, pertaining to safety and barricades.

Barricades shall have warning lights with low-intensity electric flashers for night application. See also section 1.3.2 for traffic control plan.

9 DIGGING PERMIT

9.1 To initiate a clearance/outage, the requestor must complete sections 1, 2, 6, 7 & 8 of the AF

Form 103. The requestor must attend the digging clearance/outage request meeting held every Monday at 0900 Hours in the Civil Engineer (CE) conference room in Bldg. 4197 JBSA Ft Sam Houston, AF Form FSH 103 and drawing or Map printed from the internet.

9.2 During the Clearance/Outage request meeting all digging clearance and outage request, will be scheduled and or signed off by each CE shop for their respective area of responsibility. If the area needs to have utilities located the CE shops will schedule and perform the utility locates during that week.

9.3 The requestor is also required to contact Dig Tess for all commercial utility locates. The phone number for Dig Tess is located on the digging clearance form

9.4 The requestor will be required to attend the digging clearance meeting the following Monday to have the clearance signed off by any shop that required the area to be marked. Safety, Security Forces, Fire Dept. and the Natural Resources Management Section will not attend this meeting. The requestor will be required to make contact as necessary.

9.5 Utility Outage Requests are require to be approved at least 2 weeks prior to the scheduled outage. The Requestor must notify the 502 CES Operations Flight Chief, Mr. Antony Person of the outage request once approved.

9.6 Upon completion of digging clearances or outage request the requester is required to return the completed signed clearance along with all related documents and e-mails received by the Dig Tess & commercial utilities to the 502 CES approving authority for Approval and Signature.

10 UTILITY OUTAGES

10.1. Utility outages is defined as a stoppage in electric, gas, potable water, sewer, chilled or hot water, steam, telephone, Energy Monitoring and Control System (EMCS), or any other

SOW- #CYRB110023-Malabang Trail Page Page 9 of 12 similar utility whether serving all or a part of a single facility or of several facilities.

10.2 All utility outages shall be coordinated with the Using Agency through the Contracting Officer or the 502 CES PM. Notice of the necessity for an outage shall be provided by the contractor to the Contracting Officer, in writing, not less than 14 calendar days prior to the time of the required outage and written approval for the outage will be issued by the Contracting Officer. All work shall be coordinated and arranged to ensure that the outage will be of minimum duration.

10.3 Once a utility outage is arranged and work begun, work shall continue until all utilities are restored to the affected facility or facilities.

10.4 In the event a scheduled utility outage is cancelled by the government, notification will be given to the contractor at least 24 hours in advance of the time for the outage to start.

Rescheduling of a cancelled outage must be coordinated with written notification as described above.

11 WELDING

Welding shall not be performed without first obtaining a hot work permit issued by the Base Fire Department. The contractor shall wait four to eight hours before leaving the project site or location of where the welding was taken place.

12 CONTRACTOR'S USE OF ELECTROMAGNETIC EMISSION DEVICES ON BASE

The contractor shall comply with the Air Force Military Training Center (AFMTC) Regulation 700-14, Jul 89. The regulation is available for inspection at the Communications Squadron, Building 1050, JBSA- Lackland. (Electromagnetic emission devices include: Radio/Radar Transmitters; Navigational Aids/ Instrumentations; Signaling, Intrusion Detecting, and Identification Devices; Mobile and Fixed Business Radio Communications Equipment; and MARS, CBs, and Amateur Radio Stations.)

13 RADIOACTIVE EQUIPMENT

Use of radioisotopes or radiation producing equipment (density gauging, Non-Destructive Evaluation (NDE) weld testing, etc.) requires a Nuclear Regulatory Commission (NRC) Radioactive Material License, with all documentation submitted and approved prior to bringing the material on base. To obtain authorization, coordinate the request through 502 CES PM.

13.1 The Contractor shall also comply with all requirements of AFI 40-201. Contractor shall forward application to the Contracting Officer 30 calendar days prior to commencing operations using radioactive materials.

13.2 Contractors possessing Agreement State Licenses must also submit an NRC Form 241 to NRC in compliance with 10 CFR 150.20. Request shall include the following:

a. Description of proposed activities on NRC Form 241.

b. Procedures established to ensure radiological health and safety of all personnel.

c. Name of responsible contractor representative.

d. Current copy of application for NRC or Agreement State License. The license must specifically state the installation by name or approval for temporary job sites anywhere in the United States where the NRC or Agreement State maintains jurisdiction.

e. The part of the Air Force contract describing work to be done at the base and the inclusive dates of such work.

f. An acknowledgement that the base representative can make periodic checks to ensure the contractor is following applicable radiological health and safety practices.

g. Contractors requiring more than 180 calendar days per calendar year of operation must possess an NRC license.

14 FIRE REGULATIONS AND PREVENTION PRACTICES

The contractor shall comply with fire prevention practices as set forth by the National Fire Protection Association, other recognized fire prevention agencies, and base regulations.

15 EXPLOSIVE OPERATED HANDTOOLS

The contractor shall comply with OSHA Standard 1910.215(c) when utilizing explosive operated hand tools. Storage of explosive cartridges on the installation shall be in metal containers and limited to one days' supply. The contractor shall provide adequate controls to prevent loss/theft of cartridges used and stored on the installation.

16 PREVENTION OF FUEL SPILLS

Contractor may use temporary fuel tanks upon approval by the Contracting Officer. Tanks shall be surrounded by an earthen berm which provides 110% of the tank capacity. The berm shall be lined with a 40 mil polyethylene sheet held in place with weights located on the outside of the berm. All berms shall be inspected by the 502 CES PM or the PI prior to filling fuel tanks.

17 GENERATED REFUSE

The contractor shall take positive action to prevent work-generated refuse from entering the sewer system. All contractor generated refuse and waste shall be hauled from the construction site daily to a disposal area located outside the physical boundaries of the installation or on-base when indicated. This refuse and waste material shall be hauled in trucks with tight-fitting beds and covers to prevent spillage on roadways. The construction site shall be kept neat, orderly, and safe for workmen at all times. All required manifests shall be obtained prior to the transportation of refuse off-site.

18 WATER CONSERVATION

18.1 If JBSA- implements water usage restrictions, the contractor is required to comply with these restrictions, at no additional cost to the government. In the event the contractor cannot comply with water usage restrictions and still meets other contract requirements (particularly when installing sod or hydro-mulching), then he can request permission from the Contracting Officer for relief from the restrictions.

18.2 Waivers: Submit request for waivers to water conservation restrictions in writing. The Contracting Officer will consider such cases on an individual basis and grant waivers only in extreme cases. Submit detailed requests with appropriate manufacturer’s/supplier’s data to show unavoidable noncompliance with the water usage restrictions. In the event a restriction is conditionally waived, use only the minimum amount of water necessary to comply with contract requirements.

19 INSPECTION AND ACCEPTANCE

Inspections and final acceptance will be made as completed by the contractor with the following additional provisions:

19.1 The contractor shall give the government a minimum of three (3) working days’ notice for all requested inspections. Inspections shall be staggered such that two inspections will not occur at the same time, unless it is required.

19.2 The contractor shall make sure that the work is ready for all inspections. The presence of an unreasonable number of deficiencies, as determined by the 502 CES PM, may cause the inspection to be rescheduled.

20 DETERIORATED CONSTRUCTION AND FINISH

The 502 CES PM will be the governing authority in determining if existing construction and finishes are deteriorated and require repair and/or replacement.

21 DD FORM 1354

The contractor shall prepare and submit DD Form 1354, Transfer and Acceptance of Military Real Property prior to acceptance of the project by the government. The completed DD Form 1354 shall be turned over to the 502 CES PM at least 14 calendar days prior to pre-final inspection. A final copy of the DD Form 1354 shall be submitted within 7-calendar days of final acceptance of the project.

22 OCCUPATIONAL SAFETY

The contractor shall be subject to no-notice inspections under Occupational Safety and Health Act (OSHA) Program by inspectors of the Department of Labor.

23 SEVERE WEATHER

Upon receipt by the Contracting Officer of a severe weather warning with anticipated winds of 35 knots or higher, the following sequence of actions shall be carried out:

a. Contractor is responsible for watching for severe weather warnings.

b. Contractor shall secure all materials and equipment.

c. The contractor shall take immediate action to tie down, remove, protect, and/or secure construction materials and equipment to the satisfaction of the 502 CES PI in order to reasonably assure that government property will not be damaged. If the contractor fails or refuses to secure materials and equipment to the satisfaction of the 502 CES PI, the work will be accomplished by government personnel and the cost thereof charged to the contractor.

d. Contractor shall secure their sanitary station (Port-O-John) or it will be blown over by high winds.
