About this file
This document is a Contract Amendment for Contract #CTR077260 issued by the Arizona Department of Child Safety (ADCS), specifically from the Office of Procurement & Contracts located in Phoenix, Arizona. The amendment is labeled as Amendment #1 and applies to an existing contract governing Parenting Time Services.
The amendment modifies several key sections of the original contract, primarily focusing on clarifying procedures for parenting time visits, contact attempts, and make-up sessions. Significant changes include defining a "no-call/no-show" as failing to arrive within 15 minutes of a scheduled start time, establishing guidelines for make-up sessions, and specifying that missed sessions are not automatically required to be rescheduled unless directed by an ADCS specialist. The amendment also updates the process for requesting price adjustments, requiring contractors to initiate written requests 90 days before contract extension, and becomes effective on August 18, 2025. All other terms and conditions of the original contract remain unchanged.
