About this file
This document is a Data Breach Report Form (Exhibit G) associated with Contract CTR066208 - Amendment 4 for the Arizona Department of Economic Security (ADES). The form is designed to be submitted within 24 hours of a data breach incident by a contractor, requiring detailed documentation of the breach without disclosing any personally identifying information (PII) of survivors or clients.
The form provides a structured template for reporting critical details about a data breach, including the date of the breach, date of ADES notification, a description of the incident, the organization's response, how affected survivors/clients were notified, and planned preventative measures for future data breaches. The document emphasizes strict confidentiality, instructing users to use generic terms like "Survivor" or "client" and to avoid revealing any specific identifying information. The form is intended to be submitted to the Contract Specialist and Program Specialist, with ADES committing to respond within one business day of receiving the report.
