About this file
This document is a Data Breach Report Form from Exhibit G of Contract CTR066189 - Amendment 4, specifically for the Arizona Department of Economic Security (ADES). The form is designed to be submitted within 24 hours of a data breach incident, requiring detailed information about the breach while strictly prohibiting the disclosure of any Survivor's Personally Identifying Information (PII).
The form provides a structured template for organizations to report data breaches, including sections for documenting the date of the breach, the date ADES was notified, a detailed description of the incident, the organization's response, how affected survivors or clients were notified, and planned preventative measures for future data breaches. The instructions emphasize the importance of protecting individual privacy and require submission of the form along with the organization's current Data Breach Policy. ADES commits to responding within one business day of receiving the report to confirm receipt and discuss next steps.
