About this file

This document is a Contractor Affidavit form required by Section 787.06(13) of Florida Statutes for nongovernmental entities executing, renewing, or extending contracts with governmental entities, specifically the School Board of Manatee County, Florida. The affidavit is a mandatory certification that the contractor does not engage in any form of labor or service coercion, including threats of physical force, unauthorized restraint, exploitative debt practices, document confiscation, financial harm, fraud, or controlled substance provision.

The affidavit must be signed under penalty of perjury by an authorized official of the vendor, who certifies compliance with specific prohibitions against labor exploitation. While directly related to the Artisan Lakes and Rye Ranch Elementary School Construction Management at Risk project, this document serves as a standard anti-coercion declaration to ensure ethical labor practices for contractors working with the School District of Manatee County, Florida. The form requires complete contact information for the contractor and provides space for the authorized representative's signature, affirming their commitment to fair and legal labor practices.
