About this file
This is a Foreign Countries of Concern Prohibited Affidavit required by Florida Statutes Section 287.138 for Hillsborough County Public Schools (HCPS) in connection with a Plumbing Services Request for Proposal (RFP 25200-RFP-MST). The affidavit must be completed and executed by an authorized representative of any proposer or contractor seeking to bid on or enter into a contract with HCPS. The RFP covers comprehensive plumbing services across three geographical areas in the Tampa Bay region, including general repairs, emergency services, gas line work, leak detection, water heater and boiler services, sewer line repairs, and surface restoration. Contractors must possess a valid Florida Plumbing license and maintain comprehensive insurance coverage. The proposal evaluation criteria allocates 30% for qualifications and experience, 25% for specifications and scope of work, and 35% for cost and best value, with additional points available for Small Business Enterprise (SBE) utilization. Questions are accepted until October 27, 2025, with final proposals due November 10, 2025. The initial contract term involves School Board approval with potential automatic renewals for up to four additional one-year periods.
The affidavit certifies that the proposer's legal company name does not meet any criteria that would disqualify it under Florida law, specifically that the entity is not owned by, does not have a foreign country of concern holding controlling interest in it, and is not organized under the laws of or principally based in a foreign country of concern. Foreign countries of concern are defined as the People's Republic of China, the Russian Federation, the Islamic Republic of Iran, the Democratic People's Republic of Korea, the Republic of Cuba, the Venezuelan regime of Nicolas Maduro, and the Syrian Arab Republic. The pricing sheet indicates estimated annual usage of 6,400 standard labor hours and 466 overtime hours, with additional costs for equipment rentals and call-out fees, with cost-plus markup not exceeding 15% for as-needed services. Vendors must submit pricing through an Excel spreadsheet and complete electronic bidding via VendorLink.com. The RFP anticipates most projects will be under $10,000, with potential for competitive lump sum pricing on larger projects.
