About this file
This is an affidavit form required by Florida Statutes Section 287.138 for entities bidding on contracts with Hillsborough County Public Schools (HCPS). The affidavit must be completed and signed by an authorized representative of any proposer or contractor submitting a bid, proposal, or quote for the Infrastructure Communications Failure Systems contract, which involves supplemental maintenance and repair services for Nortel/Avaya telephone systems and Honeywell Vista Series security panels. The vendor must demonstrate capability to recognize and report telephone service provider failures and coordinate repairs with other HCPS contracted vendors. Submission of the completed and executed affidavit is mandatory; bids will not be accepted without it.
The affidavit requires the contractor to certify that neither the company nor any entity with a controlling interest (defined as 25 percent or more voting interests or profit entitlement) is owned by, controlled by, or organized under the laws of designated foreign countries of concern, including China, Russia, Iran, North Korea, Cuba, Venezuela, or Syria. By signing under penalty of perjury, the authorized representative attests that the company does not meet any of the three prohibited criteria established in Florida Statutes Section 287.138(2). This certification is a compliance requirement imposed by the State of Florida to prevent governmental entities from contracting with entities having connections to foreign countries deemed threats to national security.
