Task Assignment PET 14 establishes a twelve-month period from July 1, 2017 through June 30, 2018 with a total value of $615,285.00. The contractor will perform statewide potable well surveys and sampling services on a fee basis, including 1,143 potable well surveys at $217.00 each (totaling $248,031.00) and 2,923 sampling events at $93.00 each (totaling $271,839.00), plus 1,252 out-of-county units at $23.00 each (totaling $28,796.00). Deliverables include quarterly invoices with backup documentation, quarterly CDs of completed potable well surveys, and analytical sampling results provided electronically and suitable for use in GIS and Excel databases. The funding is derived from Org. Code 37450404555 under the Florida Department of Health, with the contract amount not to exceed $615,285.00.
