About this file
Summary
This is an indefinite delivery contract (GW283) entered into on June 21, 2011, between the Florida Department of Environmental Protection (DEP) and the Florida Department of Health (FDOH). The contract includes two amendments and multiple change orders executed through July 2015. The agreement establishes a task assignment vehicle whereby FDOH provides potable water quality monitoring, well surveying, sampling, and data collection services for four DEP programs: the Petroleum Cleanup Program, Drycleaning Solvent Cleanup Program, Waste Cleanup Program, and Certification and Restoration Program.
The contract operates on a task assignment basis with compensation structured through a combination of fixed price, cost reimbursement, and fee schedule components. Water well survey services are priced at $217.00 per survey with GPS location services at $12.50 per well. Water sample collection ranges from $93.00 to $103.00 per well depending on the number of samples. Administrative services, training, travel, and equipment maintenance and purchases are reimbursable as authorized through executed task assignments. The Petroleum Cleanup Program portion is funded through the Inland Protection Trust Fund (IPTF), with separate tracking maintained for IPTF funds and other program funding sources. The contract is subject to annual legislative appropriation and remained in effect through June 30, 2016, with provisions for renewal for periods not exceeding three years. Payment processing requires five working days for contractor invoice inspection and approval, with interest accruing on late payments per Florida Statutes Section 215.422.

Sharonda;
I do not believe that their signature is required for change orders under the new procurement methods. I know that it was not required on any of the change orders that I sent in for my other contract last year. Thank You.
Sincerely

There is no signature for the procurement section on the change order that I received. Is that no longer required or did mine just get missed?
