The revision establishes new contact protocols and designates Eric Sager, Principal Geologist at Geosyntec Consultants, as the primary contact for the contractor, with notice delivery acceptable via email, U.S. Mail, courier delivery service, or electronic mail with delivery receipt confirmation. The Department of Environmental Protection's contract manager is located at 2600 Blair Stone Road, MS# 4525, Tallahassee, Florida 32399-2400, and electronic notices should be directed to the established contract manager contact. The change order was executed on October 6, 2016, and approved by the Petroleum Restoration Program Administrator.
