About this file
This is Amendment No. 12 to Agency Term Contract No. GC854 between the Florida Department of Environmental Protection (DEP) and Environmental Risk Management, Inc. The original contract was effective February 21, 2014, and governs petroleum contamination site response action services under the Petroleum Restoration Program.
Amendment No. 12 updates the Scope of Services, specifically Section 12 (Debris, Waste and Product Removal and Disposal) within Attachment A (V2.0). The amendment clarifies procedures for handling soil overages during delivery, establishing that if delivery weight tickets indicate an overage not exceeding 20% of the total volume authorized under a Purchase Order, the contractor must notify the site manager via a Request for Change within two business days of the date the additional soil was transported from the site. All other contract terms and conditions remain in effect, with the amendment provisions controlling in any inconsistencies between the original contract and this amendment. The amendment was signed by Barry Murphree, Senior VP of Environmental Risk Management, Inc., on August 9, 2022, and by John Truitt, Deputy Secretary of the Florida DEP, on August 22, 2022.
