About this file
This is Amendment No. 13 to Agency Term Contract No. GC840, a petroleum restoration program contract between the State of Florida's Department of Environmental Protection (DEP) and APEX COMPANIES, LLC. The contract was originally executed on February 28, 2014, and this amendment was signed on August 5, 2022, by Nancy Rose, Program Manager for APEX COMPANIES, LLC, and on August 22, 2022, by John Truitt, Deputy Secretary of the Florida DEP.
The amendment modifies the Scope of Services in Attachment A (V2.0), specifically Section 12 regarding Debris, Waste and Product Removal and Disposal. The update establishes procedures for handling volume overages during soil transportation and disposal operations. When delivery weight tickets indicate an overage exceeding the volume approved on a Purchase Order, notification via a Request for Change to the site manager is required within two business days of transport, provided the overage does not exceed 20% of the total volume authorized under the Purchase Order. All other terms and conditions of the original contract remain in effect, with this amendment's provisions controlling in the event of any inconsistencies.
