About this file
This document is a Vendor Performance Report (SPO 206) issued by the Arizona Department of Administration's State Procurement Office on 4/12/2023. The report involves the Arizona Department of Environmental Quality (ADEQ), specifically the Water Quality Division (WQD), and a vendor named Clear Creek Associates. The document is used to document vendor performance and potential procurement actions.
The contract (CTR045144) is for services related to properly abandoning a mineral exploratory core hole with artesian discharging low pH water. The report details multiple performance issues, including the vendor's failure to subcontract a drilling company with the proper equipment, not obtaining necessary permits, and not conducting a thorough site assessment. The procurement officer, Patricia Lorenzen, noted "No Action required, annual survey" in response to the reported performance concerns. The report serves as an official mechanism for tracking and documenting vendor performance within the state's procurement system.
