This Statement of Work is issued by the State of Illinois Department of Military Affairs on behalf of Lincoln's Challenge Academy, located at 212 W. Borman Avenue in Rantoul, Illinois. The Academy is soliciting bids from vendors to provide medical services to students attending the program. The document outlines the scope of services and operational requirements for the selected vendor.
The vendor will provide physical examinations, medical testing, and vaccinations for approximately 250 students per class, with two classes held annually—one beginning the second week of July and another beginning the second week of January. The vendor will conduct up to 500 physical examinations per calendar year on the LCA campus. Services include STD testing and vaccinations as needed, as well as TB testing for up to 20 vocational education students per year. All vendor personnel who have contact with students must complete and sign the Mandatory Vendor/Volunteer Training Agreement Form prior to conducting examinations or related services.
