About this file

This is an Environmental Clearance Report prepared by the Arizona Department of Administration, Administrative Services Division, documenting asbestos inspections conducted at the Flagstaff Lab, Building #3884, located at 1959 South Woodlands Village Boulevard, Flagstaff, Arizona. The report covers two separate inspection periods: one conducted on November 29, 2021, in preparation for roofing repair and replacement of the metal portion of the facility, and a second inspection conducted on August 27, 2025, and June 26, 2025, in preparation for roof replacement. During the 2021 inspection, thirty samples were collected from eleven suspect materials identified throughout the facility, including spray-applied fireproofing, cementitious materials, mortar, stucco, and various sealants located on the roof and mechanical penetrations. The 2025 inspection identified five suspect materials and resulted in eighteen samples collected, focusing on built-up roofing and mastic materials from roof penetrations and flashing areas. All samples were analyzed using Polarized Light Microscopy (PLM) techniques by Fiberquant Analytical Services under Asbestos Hazard Emergency Response Act (AHERA) sampling protocols.

All samples collected during both inspection periods tested negative (ND) for the presence of asbestos fibers, indicating that no asbestos-containing materials were identified in the facility. Both inspections were conducted by U.S. Environmental Protection Agency-accredited inspector Thomas Clyde, holding certification numbers ON-4644-7098-022521 (valid February 25, 2021 through February 25, 2022) and ON-188748-7098-013125 (valid January 31, 2025 through January 31, 2026). The clearance findings document that the facility is safe to proceed with the planned roof replacement and repair work without requiring asbestos abatement procedures.
