Section B. CURRENT CONDITIONS

B.1. OVERVIEW & BACKGROUND

CMS oversees the administration of group health benefits for four separate and distinct Programs (SEGIP, TRIP, CIP, and LGHP), which collectively cover approximately 450,000 lives. The statutory authority for the Programs is the State Employees Group Insurance Act of 1971, as amended, 5 ILCS 375/1 et seq. (available at https://www.ilga.gov/legislation/ilcs/ilcs3.asp?ActID=120&ChapterID=2 ), hereinafter referred to as the "Act". Each Program has its own plan design and funding source; therefore, each Program must remain mutually exclusive for all reporting packages, financials, reports, claims payments and rates.

The Act defines and establishes base criteria for those eligible to participate in each Program. The current statutory definition, or as amended in the future, will prevail on all questions of eligibility to participate.

Eligibility Currently, CMS determines member eligibility in accordance with the Act and the information is provided to the Benefits Administration vendor through a daily file transfer. It is the responsibility of the current Benefits Administration vendor to furnish original enrollment information and weekly updates of changes to all contracted insurers and third-party administrators (TPAs), collectively referred to as the “Carriers”. The Carriers must accept all determinations of eligibility for plan participants as determined by CMS and transmitted to them by the Benefits Administration vendor. Member election, dependent biographical and enrollment data, and contribution data is maintained by the Benefits Administration vendor. It is possible that members may be eligible for and/or enrolled in more than one Program (e.g., enrolled in Life Insurance only through SEGIP and enrolled in health coverage through TRIP).

Additional Information for LGHP participating units:

· Qualified units of local government (“LGHP Unit”) can elect to join LGHP at any time. If an LGHP Unit begins participation in LGHP at any time other than the beginning of a fiscal year, the Unit’s first plan year will extend only until the end of the fiscal year in which participation began.

· Each LGHP Unit has unique plan premiums that are determined by CMS.

· Plan Participant contributions are determined by and vary by the applicable LGHP Unit; there are currently about 50 combinations of participant premium contributions.

· Each LGHP Unit currently receives unique billing statements from CMS.

· LGHP currently includes about 2,750 participants from 201 LGHP Units.

· LGHP Units are offered the same medical plan carriers and medical plan types as SEGIP, CIP, and TRIP; however, each Program has unique plan designs for each plan type.

Annual Benefit Choice/Open Enrollment

The Director of CMS determines the Benefit Choice period(s) and/or Open Enrollment period(s) to allow plan participants to change health plans or other benefit options. Such period occurs at least once per year, usually May 1 – May 31 for the traditional plans and Oct 15-Nov 15 for the MAPD Plans.

Coverage and Premium Adjustments

Retroactive changes in elections and resulting premium adjustments are allowed for a period of up to six months from the month in which the change is reported. At the discretion of CMS, exceptions to this retroactive adjustment policy may be made for administrative reasons or in order to comply with the Act or court action. Health plans currently provide for a continuation and conversion privilege for plan participants in accordance with Section 6.2 of the Act (5 ILCS 375/6.2) and Sections 367.2, 367e, and 367e.1 of the Illinois Insurance Code (215 ILCS 5/367.2, 5/367.e and 5/367e.1).

Benefits Administration Solution The State’s benefits administration solution will include the key elements described below. The State may modify these elements at its discretion or as required by collective bargaining agreements (CBA), statutory requirements, or the State’s contracts with its Carriers. Detailed services to be provided by the selected vendor are identified in the solicitation. The benefits administration solution should provide at least the following key elements:

· The selected vendor must work with the current Program Carriers for purposes of administering benefits.

· SEGIP interfaces with approximately 23 payrolls and three (3) retirement systems for deduction processing. The Agency will disburse payroll information to each of the payroll and retirement systems.

· The State’s entire covered population (both Medicare and non-Medicare) will continue to be offered their current health plan options.

· Carriers will vary by geography (County)

· Each Program’s plan designs will be defined by the State and uniform across Carrier type (e.g., HMO, PPO, OAP), where offered.

· The State will continue to contract directly with all Carriers for each Program.

· The State intends to continue to establish the premiums, self-insured premium equivalent rates, Participant contributions and/or agency reimbursements and State subsidies for all plans.

· The State’s current group life and dental coverages will be administered through the benefit solution as exclusive offerings. Vision coverage, when applicable, is at no additional member cost.

· The selected vendor shall contract for and provide select, non-competing voluntary coverages to be purchased on a post-tax payroll deduction or direct billed basis by SEGIP, CIP, and TRIP plan participants. The State will determine which voluntary coverages may be offered to plan participants.

· LGHP Units will not be eligible to purchase voluntary coverages on a payroll deducted basis.

· The selected vendor will provide eligibility, enrollment, and benefits administration services for all Programs as outlined in the Statement of Work.

B.2. VOLUMES, QUANTITIES & SYSTEM INFORMATION

B.2.1. Please refer to Appendix 1 for volume, quantities, and additional background data.

B.2.2. Please refer to Appendix 2 for Group Insurance Reimbursement System information.

B.3. AREAS OF CONCERN/CHALLENGES

B.3.1. The State’s Benefit Programs are generally more complex given the following:

· Must provide health plan coverage nationwide and must have an HMO in every county in Illinois.

· The number of programs administered (SEGIP, TRIP, CIP and LGHP)

· Each of the four programs offer different plan designs.

· The number of plan offerings (PPO, HMO, OAP and MAPD)

· The number of statutory requirements impacting the programs and plans

· Each of the four programs have different:

· Funding mechanisms

· Budgetary challenges

· Claim payment delays for the CIP Program (claim hold)

· Numerous and diverse Collective Bargaining Agreements (CBA)

B.3.2. The State is requesting that the selected vendor be able to provide split family coverage wherein Medicare-eligible plan participants enroll in an MAPD Plan, while the non-Medicare plan participants in the family unit remain in one of the State’s traditional medical plans. The State requires the family unit stay “connected” to one another for purposes of eligibility but would like to allow the flexibility of enrollment into different medical plans. Currently Medicare-eligible coverage transitions occur when the last member of the household becomes Medicare eligible.

B.4. STRENGTHS

Implementation of the benefits administration platform has resulted in administrative efficiencies throughout the State at various agencies, universities, and retirement systems due to the decrease in manual workload of Group Insurance Representatives (GIRS) and payroll processors and has eliminated the risk of manual manipulation of enrollment documents.

CMS currently contracts with Telus Health to provide full-service Benefits Administration services for the State’s health, dental, vision, life and flexible spending account benefits. Services include but are not limited to the following:

· On-line enrollment portal

· On-line and/or call center services for Members and the 500+ insurance liaisons within each agency, board, commission, universities and retirement systems

· Member communications for benefit elections, including calculation and transmission of member premiums for payroll deductions

· Creation of direct billing statements for members on leave of absence, COBRA, local government units, etc.

· Integrated with Illinois ePay for certain member payment purposes

· Transmission of member elections/eligibility files to contracted vendors

· Calculation and reporting of premiums due to carriers/vendors and other reporting

· Rolling MAPD enrollment

· COBRA administration

· Affordable Care Act (ACA) compliance administration

Offers become the property of the State. All offers will be open to the public under the Illinois Freedom of Information Act (FOIA) (5 ILCS 140) and other applicable laws and rules, unless Offeror requests in its Offer that the State treat certain information as confidential. A request for confidential treatment will not supersede the State’s legal obligations under FOIA. The State will not honor requests to keep entire Offers confidential. Offerors must show the specific grounds in FOIA or other law or rule that support confidential treatment. Regardless, the State will disclose the successful Offeror’s name, the substance of the Offer, and the price.

If Offeror requests confidential treatment, Offeror must submit additional copy/copies (see Instructions for Submission of Offers in Section C.) of the Offer with proposed confidential information redacted. This redacted copy must tell the general nature of the material removed and shall retain as much of the Offer as possible. In a separate attachment, Offeror shall supply a listing of the provisions identified by section number for which it seeks confidential treatment and identify the statutory basis or bases under Illinois law, including a detailed justification for exempting the information from public disclosure.

Offeror will hold harmless and indemnify the State for all costs or damages associated with the State defending Offeror’s request for confidential treatment. This competitive process may require that Offeror provide additional information and otherwise cooperate with the State. If an offeror does not comply with requests for information and cooperate, the State may reject the offer as non-responsive to the solicitation. Submitting an offer does not entitle Offeror to an award or a contract. Posting a vendor’s name in a Bulletin notice does not entitle the vendor to a contract. The State is not responsible for and will not pay any costs associated with the preparation and submission of any offer.

Awarded Offeror(s) shall not commence and will not be paid for any billable work undertaken prior to the date all parties execute the contract, unless approved in writing in advance by the State Purchasing Officer or the Chief Procurement Officer (or designee).

If the State issues an award, the award will be made to the responsive and responsible offeror whose offer best meets the specified criteria unless otherwise permitted by the Illinois Procurement Code and Illinois Administrative Code. However, if the State does not consider the price to be fair and reasonable and negotiations fail to meet an acceptable price, then the State reserves the right to cancel the award and take appropriate action to meet the needs of the State.
