About this file

This document contains a past performance questionnaire template for evaluation of contractor performance on a previous government contract. The questionnaire requests information on the contractor's managers and supervisors, ability to hire and replace personnel, delegation of authority, quality control procedures, timeliness, problem identification and resolution, scheduling, responsiveness to changes, supervision, surety consent for increased work, timely payment of subcontractors, cost proposals, and cooperation with government personnel. Respondents are asked to rate the contractor's performance on a scale of 1 to 6 in these areas, and provide narrative explanations for ratings of 1 or 2. The questionnaire also inquires if the contractor received cure or show cause notices on the contract, and whether the respondent would award the contractor another contract, with explanations required for negative answers.
