About this file

This is a SCION Data Flow Document providing a comprehensive architectural overview of the South Carolina Infectious Disease and Outbreak Network system used by the South Carolina Department of Public Health (DPH). The document outlines the integrated system architecture for disease surveillance and outbreak governance, tracking infectious diseases including COVID-19, sexually transmitted diseases, Hepatitis, and Tuberculosis across the state. The SCION system integrates both internal DPH applications and external data sources to manage the flow of disease surveillance data, utilizing multiple data transmission standards and protocols including HL7, Electronic Laboratory Reporting (ELR), Clinical Document Architecture (CDA), and Health Level Seven standards. The system receives data from various sources including hospitals, external private laboratories, electronic health records through the South Carolina Health Information Exchange (SCHIEx), and manual data entry, with outputs directed to the CDC's National Electronic Telecommunications System for Surveillance (NETSS) and other federal reporting mechanisms.

The system architecture employs multiple data delivery mechanisms including PHINMS (Public Health Information Network Messaging System), AIMS (APHL Informatics Messaging Services), CSV file transfers, SOAP and RESTful web services, and SFTP connections to manage bidirectional data flow. Key system components include the Maven 6.3 integration platform, Rhapsody v6.3 for message processing, the OpenELIS Laboratory Information System, and a custom web-based SCIONx portal for provider test result data entry. The document establishes a business glossary of 20 technical acronyms and system components relevant to electronic disease surveillance and interoperability, providing the technical framework for how infectious disease data is collected, transmitted, analyzed, and reported throughout South Carolina's public health surveillance infrastructure.

SCION Data Flow Document

Scope: This document provides a comprehensive architectural overview of the South

Carolina Infectious Disease and Outbreak Network, SCION, using both internal and external components to depict how data flows into and out of the system. The diagram is intended to capture the architecture of the system.

It is not, however, an in-depth technical detail as to the paths or different formats data can take between major applications.

Business Glossary and Acronyms:

• AIMS-APHL Informatics Messaging Services: a secure, cloud-based platform that accelerates the implementation of health messaging by providing shared services to aid in the visualization, interoperability, security and hosting of electronic data.

• APHL- Association of Public Health Laboratories: a US-based membership organization made up of governmental public health laboratories whose mission is to detect hazards and generate scientific data to inform public health action.

• CDA-Clinical Document Architecture: a document markup standard that specifies the structure and semantics of "clinical documents" for the purpose of exchange between healthcare providers and patients.

• CELR-COVID Electronic Laboratory Reporting: a secure, electronic pathway created by the CDC for COVID data reporting by laboratories.

• CSV -Comma Separated Values: a text file format that uses commas to separate values.

• el CR -Electronic Initial Case Report: a consensus-based Health Level Seven International

(HL7) standard developed for use in electronic Case Reporting (eCR) based on the HL7

Clinical Document Architecture. Includes Council of State Territorial Epidemiologist (CSTE) identified data elements necessary for public health to initiate a case investigation.

• ELR -Electronic Laboratory Reporting: the transmission of digital laboratory reports, from laboratories to state and local public health departments, healthcare systems, and CDC.

• ETOR-Electronic Test Orders and Results: a secure multi-directional platform that transmits electronic test orders and results between healthcare facilities and Public Health

Labs (PHLs).

• HL7 -Health Level 7: a range of global standards for the transfer of clinical and administrative health data between applications.

• LIS-Laboratory Information System: an application to streamline the management of laboratory processes including data collection, workflow management, and report generation. Also referred to as a Laboratory Information Management System (LIMS).

• MIF - Maven Integration Format: XML schema for Maven data importer

• NETSS-National Electronic Telecommunications System for Surveillance: CDC system for collecting, transmitting, analyzing, and publishing weekly reports of notifiable diseases and injuries from the 50 states, New York City, the District of Columbia, Puerto Rico, the Virgin

Islands, Guam, American Samoa, and Commonwealth of the Northern Mariana Islands.

• NND-Nationally Notifiable Disease: a disease that, when diagnosed, requires health providers to report to state or local public health officials. Notifiable diseases are of public interest by reason of their contagiousness, severity, or frequency.

• PHINMS-Public Health Information Network Messaging System: the primary data transport mechanism for the CDC's Public Health Information Network.

• RR-Reportability Response: a consensus-based Health Level Seven International (HL7) standard developed for use in electronic Case Reporting (eCR) based on the HL7 Clinical

Document Architecture. This is a response file for elCR authors.

• SAMS-Secure Access Management Services: a federal information technology (IT) system that gives authorized personnel secure access to non-public CDC applications. This is a

CDC Secure External Partner Portal.

• SCHIEx-South Carolina Health Information Exchange: a secure network that provides physicians with statewide, real-time access to a patient's medical history.

• SCION- South Carolina Infectious Disease and Outbreak Network: an integrated system for disease surveillance and outbreak governance. SCION is used to track infectious diseases in the state, including COVID, STDs, Hepatitis, and Tuberculosis.
