About this file
This Amendment 4 document modifies the Request for Proposal for an Electronic Disease Surveillance System issued by the South Carolina Department of Public Health through the State Fiscal Accountability Authority's Division of Procurement Services. The solicitation requests development and implementation of an electronic disease surveillance system, with offers to be submitted online through the SCEIS portal by March 5, 2025 at 11:00 AM ET. A non-mandatory pre-proposal conference was held on October 21, 2024 at 10:00 AM ET via Microsoft Teams. The deadline for offerors to submit written questions was extended to January 3, 2025 at 12:00 PM ET. Award is scheduled to be posted on May 2, 2025, and offerors must hold their offers open for a minimum of 120 calendar days following the opening date.
Amendment 4 clarifies multiple specifications regarding pricing and milestone submissions, specifically addressing the format and content requirements for Attachment J, which contains the Total Cost Summary and Milestone worksheets. Offerors are instructed to complete cost projections through Line 32 to capture annual recurring operations and maintenance costs, with milestone completion timelines measured in weeks rather than specific dates. Offerors claiming in-state preferences must provide an in-state office address. All qualified offerors must submit comprehensive qualifications documentation with their proposals, including financial statements, detailed project references, business history, and disclosure of any failed projects or litigations. Bidders proposing to submit consolidated financial statements from a parent company must include their DUNS number with the offer.
