About this file

This is a Conflict of Interest Affidavit document for the State of Maryland, specifically related to snow removal services at Spring Grove Hospital Center. The affidavit requires all offerors, contractors, consultants, and subcontractors to disclose any conflicts of interest that could impair their objectivity or render them unable to provide impartial assistance to the State. A conflict of interest is defined as situations where other activities or relationships prevent impartial performance of contract work or create an unfair competitive advantage. The affidavit mandates that offerors warrant the absence of relevant facts or circumstances giving rise to conflicts of interest, except as disclosed in the document itself.

The affidavit requires signatories to immediately notify the procurement officer in writing if any actual or potential conflict of interest arises after the affidavit's execution date, including a detailed description of all relevant facts and actions taken to avoid, mitigate, or neutralize the conflict. The document must be completed under penalty of perjury by an authorized representative and submitted with the bid or proposal. The affidavit is effective as of July 2024 and references COMAR 21.05.08.08 and COMAR 21.01.02.01B(64) for regulatory framework and definition of "person" in the context of state procurement requirements.
