Attachment G - Responsibility Inquiry for Oregon BUYS Solicitation #S-33000-00017127; Agency RFQ #26-099 SEED

This is a Responsibility Inquiry form used by the State of Oregon to evaluate the qualifications and financial capability of Energy Analyst Firms prior to award of a prequalified list agreement. The Agency will assess whether firms possess the appropriate financial, material, equipment, facility, and personnel resources necessary to meet all contractual responsibilities. The form requires firms to disclose their performance history over the last five years, specifically the number of similar contracts completed on time and within budget without claims, as well as any contracts that failed to meet those standards.

The form includes extensive vetting requirements covering criminal and civil charges within the last three years, contract terminations for default, pending litigation, outstanding judgments, financial distress, cash flow adequacy, bankruptcy filings, and required licenses and registrations to conduct business in Oregon. Firms employing 50 or more full-time workers on contracts exceeding $500,000 must provide a current Pay Equity Certificate from the Department of Administrative Services. The authorized representative must certify under signature that all responses are complete, accurate, and not misleading. Failure to promptly provide requested documentation or demonstrate responsibility may result in a finding of non-responsibility and rejection from the prequalified list.
