About this file

This is an Information and Certification Sheet for Oregon Buys Solicitation #S-33000-00017127, Agency RFQ #26-099 SEED, issued by the State of Oregon for Energy Analyst Firm qualification. The solicitation seeks to establish a prequalified list agreement for energy analyst services. Respondents must complete this certification form as part of their Statement of Qualifications (SOQ) submission. The SOQ remains valid for 180 days following the opening of the solicitation. Upon award of a prequalified list agreement, selected firms agree to perform work and meet performance standards as defined in the final negotiated statement of work included in Attachment A. The form requires firms to provide basic company information including legal name, address, state of incorporation, entity type, contact information, and Oregon Business Registration Number.

The certification sheet establishes mandatory compliance requirements for participating firms, including adherence to the Prequalified List Agreement terms and conditions, payment of all applicable state and local taxes, non-discrimination in employment and subcontracting practices, and compliance with wage and hour laws under ORS 652.220. Firms with anticipated contract values of $150,000 or more must certify possession of written policies preventing sexual harassment and assault. Additional certifications require that firms verify they are not on federal exclusion lists, have no actual or potential conflicts of interest with the State, and that all SOQ contents are truthful and prepared without collusion. Firms must also acknowledge potential liability under the Oregon False Claims Act for any false statements or material omissions in their submissions.
