About this file
This is a Contract Monitoring Division (CMD) Form 2A submission document used by the City and County of San Francisco for Architecture, Engineering, and Professional Services contracts. The form is mandatory for all proposals and requires proposers, joint venture partners, subconsultants, vendors, and lower-tier subcontractors to be listed with detailed participation information. Proposals lacking this completed form may be deemed non-responsive and rejected. The form captures the scope of work for each participating firm, percentage allocation of work, and Local Business Enterprise (LBE) classification status. The related Department of Public Health (DPH) Cost Accounting and Decision Support System solicitation is being issued by the City and County of San Francisco and seeks qualified firms to provide the City with a cost accounting and decision support system to analyze cost structure and perform cost and revenue analysis. The RFP was published on September 8, 2026, with a pre-proposal conference scheduled for September 21, 2026, at 10:00 AM PST. The deadline for submitting questions is September 28, 2026, at 5:00 PM PST, with a Q&A addendum to be posted on October 7, 2026. Proposals are due by October 19, 2026, at 2:00 PM PST. Short-listing notifications are estimated for November 16, 2026, with demonstrations scheduled between November 23 and December 11, 2026. The award is anticipated on December 21, 2026, with a Health Commission presentation if applicable in April 2027 and an estimated contract start date of May 1, 2027.
The CMD Form 2A establishes LBE sub-participation requirements that must be met by all proposers, with the specific requirements to be stated in the RFP/RFQ. LBE proposers and joint ventures with LBE participation must satisfy applicable sub-participation thresholds and cannot count their own participation toward meeting these requirements. Rating bonuses are available depending on contract value and LBE composition: contracts estimated between $400,000 and $10,000,000 offer bonuses ranging from 5% to 10% for Micro or Small-LBE participation or Joint Ventures, while contracts exceeding $10,000,000 but not exceeding $20,000,000 offer a 2% bonus for Micro, Small, or SBA-LBE participation. Additional rating bonuses up to 1.5% are available through the Neighborhood/Zip Code LBE Program for projects within San Francisco's jurisdictional boundaries, excluding Job Order Contracts and as-needed contracts. The Mentor-Protege Program rating bonus is applicable only for Design-Build and CM/GC projects. All firm information, including vendor numbers, Federal ID numbers, and service descriptions, must be provided in Section 3 of the form.
