About this file
This document is a Task Order Procedure and Form template issued by the Los Angeles County Metropolitan Transportation Authority (Metro) that establishes the operational framework for contractors performing services under awarded contracts. The procedure outlines a structured process wherein Metro's Project Manager initiates task orders by providing a location, service description, completion deadline, and delivery requirements to the contractor. Upon receipt, the contractor must submit a completed and signed task order form with a firm fixed price proposal within one business day, including a detailed breakdown of quantities, unit rates, and subtotals for all services and parts. Within two working days of receiving a fully executed task order from the PM, the contractor must submit a detailed work plan and schedule for Metro's review and approval. The PM retains authority to issue or revise task orders during contract performance, subject to contractor acceptance, with specific delivery timeframes defined in individual task orders.
The pricing structure requires contractors to provide cost breakdowns across labor rates, labor hours, materials, and other expenses, with a not-to-exceed amount specified for each task order. Contractors must identify their Small Business Enterprise/Disadvantaged Veterans Business Enterprise commitment percentage for each task order. All task orders are subject to Metro audit upon completion of services, and payment is made according to the contract's payment provisions with supporting documentation required at invoice submission. The template includes approval signature lines for Metro's Senior Manager, Senior Financial Management Services, and Financial Management Services representatives, as well as a contractor acknowledgment and acceptance signature line.
