About this file

This is a Certification of American-Manufactured Goods form issued by the Maryland Department of General Services (DGS) for use in state solicitations and contracts. The form requires bidders and offerors to certify the manufacturing origin of goods offered under a solicitation or contract, specifically related to the Sand and Spring Run Subsidence Repair project (F-001-260-004). Bidders must select one of two options: certify that all offered goods are American-manufactured goods as defined in COMAR 21.11.15.04B(1), or certify that offered goods are not American-manufactured and identify which items fall under emergency life safety and property safety goods versus non-emergency goods. The certification must be executed by an authorized representative of the bidder/offeror with their signature, printed name, title, and date.

The form includes two attachments for documentation purposes. Form A requires bidders selecting Option 2 with subsection (a) to list all goods that are not American-manufactured but qualify as emergency life safety and property safety goods, with corresponding specification sections or drawing references. Form B requires listing of goods that are not American-manufactured and do not qualify as emergency life safety and property safety goods, also with specification references. Bidders may attach additional sheets as needed to accommodate all applicable goods. This certification is a mandatory compliance document for the Maryland procurement process and reflects the state's preference for American-manufactured goods in public contracting.
