About this file
This is a Professional Technical & Expert (PTE) Services Participation Disclosure Form issued by the City of Portland for use in response to a Request for Proposal (RFP) related to Debris Management Removal Services in Oregon. The form is mandatory for all proposers and must be submitted with proposals; failure to submit the form may result in the proposal being deemed non-responsive and rejected from consideration. Proposers are required to disclose comprehensive information about all contract participants, including the prime contractor and all subconsultants, along with their roles, work scopes, and percentages of total contract work.
The disclosure form requires proposers to provide contact information, Employer Identification Numbers (EINs), State of Oregon Disadvantaged, Minority, Women, and Emerging Small Business (D/M/W/ESB) certification status, the percentage of work each firm will perform, and the percentage of the total contract amount allocated to Oregon-certified D/M/W/ESB subconsultants. Proposers must verify D/M/W/ESB certification status through the State of Oregon's official database and cannot use placeholder terms such as "TBD" or "N/A" for any required fields. Even proposers not utilizing subconsultants must complete the form by entering their own information and indicating "NONE" in the subconsultant section. The form includes multiple sections for subconsultant information to accommodate firms utilizing various subcontractors on the project.
