About this file

This is an Attestation Form (Attachment J) for an Indiana state and local contract opportunity solicitation. The form serves as a mandatory submission document that requires respondents to confirm completion and submission of all required proposal components, including the Executive Summary, Business Proposal, Technical Proposal, Cost Proposal, and supporting documentation such as the Indiana Veteran Owned Small Business form and Artificial Intelligence Technical Proposal Questions. Respondents must designate an Internal Response Manager who will serve as the dedicated contact for all State correspondence related to the solicitation and must provide their name, email address, and confirm their legal authorization to commit the organization contractually through submission of corporate bylaws or board resolutions.

The attestation form requires respondents to confirm their understanding of mandatory submission requirements, contract terms and clauses, subcontractor provisions, and confidentiality restrictions outlined in the RFP. Respondents must actively claim Buy Indiana certification status if applicable, with the requirement that certification be finalized at the time of submission; the form provides guidance that Indiana business designation requires either a principal place of business in Indiana, majority payroll to Indiana residents, majority Indiana employment, substantial capital investment of $5 million or more in plant/equipment, or annual lease payments of $2.5 million or more, or ranking in the top 500 companies by employees, unemployment taxes, payroll withholding taxes, or Corporate Income Taxes. The form also addresses qualified state agency preference scoring for nonprofit agencies serving persons with severe disabilities, Secretary of State registration requirements, and procedures for claiming confidential or redacted documents under Indiana's Access to Public Records Act (APRA) with specific formatting requirements for submission.
