Attachment 2-C Facility Location & Staffing Chart (Amended 4.16.25) If your facility has specific holidays or days when it will not be open, please detail in an attached document how you will maintain service availability to meet contract requirements during these times. Offeror Response:
Our Visitation Center is open Monday-Sunday from 7:00 AM to 8:00 PM including holidays and weekends. Our Administrative staff is available Monday through Friday, 8:00 AM to 5:00 PM. Our Case Aide’s have 24 hour access to the visitation center and use the center to provide visitations as needed or as determined by ADCS, including holidays, if requested by the participants. This allows us to meet contract requirements and maintain service availability for our participants.
For designated holidays, visitations for families are coordinated with our Case Aides for either the visitation office, community visits or home visits and may still occur as agreed upon by participants, Case Aides, and Case Managers. During visitations that may happen on holidays, where an assigned Case Aide is not available, we utilize part-time and on-call staff to manage cases so there is not a disruption to services. These visits are scheduled individually, case by case, and Administrative oversight is available as needed. Our Administration is on call at all times to provide direction and feedback to Case Aides as needed.
