About this file

This is an Affidavit of Eligibility Form for the Opportunity Zone Program administered by the Central Florida Tourism Oversight District in Florida. The form is used to certify that a business meets eligibility requirements for participation in a federal economic development initiative created under the Tax Cuts and Jobs Act of 2017. The program targets businesses with main offices located within designated Qualified Opportunity Zones in Orange, Osceola, Lake, Polk, and Seminole counties. Eligible businesses must complete and submit this affidavit when responding to solicitations related to the ECEP Cooling Tower Basin Improvements project, particularly if they are not already certified under the program. The form requires businesses to provide their legal name, main office address, the specific county where the main office is located, and an authorized signature attesting to their location within an eligible Opportunity Zone.

The affidavit serves as a mechanism to encourage bidding and contracting participation from businesses located in economically distressed communities within the specified Florida counties. Businesses seeking to qualify for consideration under this program set-aside must verify their eligibility through the official Opportunity Zones maps available on FloridaJobs.org before submitting their response. No specific pricing terms, contract amounts, or award dates are detailed in this eligibility form, as it functions solely as a certification document to establish business qualification status for participation in opportunity zone contracting initiatives.
