About this file

The document is a Contract Change Order/Amendment from the Arizona Department of Corrections (ADOC), dated July 7, 2017, for Contract No. ADOC16-113022 / ADC No. AD9-010-A3-001. Amendment No. 26 modifies an existing contract, specifically addressing the Scope of Work related to Applicant Background Checks. The amendment is associated with the State of Arizona's corrections system and was prepared by Anna Mroz, a Senior Procurement Specialist.

The amendment requires the Facility Operator to complete an Application Record Notification form along with DO 602 Background Investigation documents, which must remain on record at the facility. The document outlines procedures for criminal history record checks through the Federal Bureau of Investigation (FBI) and provides guidance for applicants to review, challenge, or correct their criminal history records. The amendment emphasizes the applicant's rights to be informed about and potentially dispute information in their criminal history before any adverse action is taken. All other contract terms and conditions remain unchanged, with the amendment becoming effective upon acknowledgment in the ProcureAZ system.

Applicant Record Notification

Applicant Notification and Record Challenge

Your fingerprints will be used to check the criminal history records of the Federal Bureau of

Investigation (FBI). If you have a criminal history record, the officials making a determination of your suitability for the job, license, or other benefit must provide you the opportunity to complete or challenge the accuracy of the information in the record. You should be afforded a reasonable amount of time to correct or complete the record (or decline to do so) before officials deny you the job, license, or other benefit based on information in the criminal history record.

The procedures for obtaining a change, correction, or updating of your FBI criminal history record are set forth in Title 28, Code of Federal Regulations (C.F.R.), Section 16.30 through

16.34. Information on how to review and challenge your FBI criminal history record can be found at www.fbi.gov under ‘Services’– ‘Identity History Summary Checks (IdHSC)’ or by calling (304) 625-5590.

To obtain a copy of your Arizona criminal history to review, update, or correct the record, you can contact the Arizona Department of Public Safety (DPS) Criminal History Records Unit at

(602) 223-2222 to obtain a fingerprint card and a Review and Challenge packet. Information on the Review and Challenge process can be found on the DPS website at www.azdps.gov.
