About this file
This addendum modifies a Request for Qualifications solicitation issued by the City of Phoenix Finance Department's Central Procurement Division for critical incident psychological services to be provided to the Phoenix Police Department. The addendum extends the proposal due date to October 18, 2022, at 2:00 p.m. and establishes a five-year contract term commencing on or about November 1, 2026. The solicitation requires comprehensive post-critical incident support, counseling, consultation, trauma-related mental health services, and training for law enforcement personnel. Required qualifications include clinical psychologists or master's level clinicians licensed in Arizona with trauma-related mental health experience specific to public safety and a minimum five-year business operation history. The contractor must provide 24/7 emergency response capability within two hours, maintain a base of operations within the Phoenix metropolitan area, and deliver one wellness check annually for all sworn personnel and two wellness checks for high-risk unit personnel. A non-mandatory pre-offer conference was conducted on April 24, 2026, with the written inquiry deadline of May 1, 2026, and original proposal responses due May 12, 2026, at 2:00 p.m., though this addendum extends that deadline to October 18, 2022. Offerors must acknowledge receipt and acceptance of this addendum through the City's Procurement Portal.
Pricing will be firm and fixed for the initial one-year period, with annual price increase requests permitted thereafter with 60 days' notice and supporting documentation. Insurance requirements include commercial general liability of $2,000,000, automobile liability of $1,000,000, professional liability of $2,000,000, and workers' compensation at statutory limits. Evaluation criteria are weighted as follows: experience and qualifications (50 percent), method of approach (37.5 percent), and fee schedule (20 percent). No incumbent vendor or current contractor is identified. Contractors must comply with Disadvantaged Business Enterprise requirements, prompt payment to subcontractors within seven days of receipt of payment from the City, the Immigration Reform and Control Act of 1986, and Arizona's legal worker requirements verified through E-Verify. Contract documents must be submitted through the City's Procurement Portal, and authorized representatives are required to sign a Sensitive Security Information Acknowledgement Form. The contract award will be made to the responsive and responsible offeror whose offer is determined most advantageous to the City based on stated evaluation criteria, with the contract terminating upon the earliest occurrence of reaching the end of the term including any extensions exercised or termination pursuant to the provisions of the agreement.
