About this file
This document is a Price Proposal Template for an Electronic Health Record (EHR) software system Request for Proposal (RFP) #MW26-3, issued by the State of Utah Division of Purchasing. The RFP seeks an EHR software system for multiple governmental and public entity settings, including the Department of Health and Human Services Correctional Health Services, local County Jails, Juvenile Justice & Youth Services, and other potential governmental entities. The contract aims to provide a standardized EHR system across correctional facilities, with the ability to bill Medicaid, and is partially prompted by Utah HB39 from the 2025 legislative session.
The pricing template provides a comprehensive breakdown of implementation, origination, and annual fees structured in tiered levels based on user count, admin licenses, provider licenses, and inmate bed capacity. The proposal allows for fees across a ten-year period, with detailed pricing categories including system configuration, data migration, licensing, hosting, technical support, maintenance, and upgrades. Hourly fees are also included, categorized by labor skill levels, with an instruction that billable time will be tracked in 15-minute increments and should include all-inclusive overhead costs. The contract is designed as a cooperative contract that can be utilized by eligible users beyond correctional facilities, with each user having the choice to opt into the contract.

HOURLY FEES
For additional work (as pre-authorized in writing by the Eligible User) not covered in the "implementation origination fees" and "annual fees" sections above Instructions for this "hourly fees" section:
For the purposes of this section, "hourly fees" should be “all inclusive” of other overhead costs. This includes, but is not limited to: the Proposer’s standard overhead costs (office space, utilities, administrative employees, supervision, insurance, licenses, permits, bonds, taxes, and expenses that are involved in running the business); the cost of tools and equipment to do the work; the cost to appropriately dispose of waste; travel time; time spent shopping for and/or ordering supplies / materials; software; hardware; and contingency costs. In calculating your proposed hourly fees, you should also take into consideration your company’s desired profit margin. Billable time will be limited to time spent working on the project. Time will be billed in 15-minute increments.
| Work to be performed by someone with the following level of skill / experience | Description | Proposed, MAXIMUM | ||
| “all-inclusive” hourly rate for work performed during normal business hours, without overtime | Proposed, MAXIMUM |
“all-inclusive” hourly rate for work performed outside of normal business hours, which may or may not include overtime
| Level one labor | Workers in this category meet the bare minimum requirements to perform the work but may need frequent quality checks and guidance from more experienced workers in order to consistently meet the company’s quality standards. They may still be working on improving their speed while still maintaining quality. They can successfully complete the most common work but would not yet be assigned independent work that really takes superior skill. |
| Level two labor | Workers in this category have the skill and experience to work independently and consistently meet the Company’s expectations for both quality and speed. This worker has the skill and experience to be assigned work that really takes superior skill. |
| Level three labor | Workers in this category have the skill and experience of the level two worker and are able to supervise and/or train the company’s other workers. |
For evaluation purposes the hourly fees listed above will be weighted by 300. Total hourly fees after being weighted: $0.00
Total (Total Implementation Origination Fees, cell E16) + (Ten-Year Cost of Annual Fees, cell O41) + (Total weighted hourly fees, cell I51) = $0.00 Total that will be compared to other offerors' pricing proposals
