About this file

This document is an Attachment G Example Scoresheet for an Electronic Health Record (EHR) software system Request for Proposal (RFP) # MW26-3, designed for correctional facilities. The RFP outlines a comprehensive evaluation process for selecting an EHR software solution, with multiple stages of technical and cost assessment. The proposal requires a core electronic health record system that has been in active use for a minimum of two years by at least one governmental entity or major medical organization in the United States, with fundamental EHR functionalities including patient registration, scheduling, clinical documentation, and medication management.

The evaluation process is structured across five stages, with a total of 1,000 possible points. The technical evaluation comprises 85% (850 points), including mandatory requirements, proposed terms and conditions changes, and scoreable technical criteria assessed through written proposals and software demonstrations. The remaining 15% (150 points) is allocated to cost evaluation. Bidders must meet minimum technical point thresholds in stages 3 and 4 to progress, with specific minimum point requirements for various technical criteria such as "most important" features (45 points), security/compliance features (15 points), and overall technical evaluation (162.5 points at 65%). Proposals not meeting these thresholds will be deemed "not responsive" and ineligible for further consideration.
