About this file

This file is a Life Disability Questionnaire submitted by the City of Burleson in response to the Ancillary Insurance Benefits contract opportunity in Texas. The questionnaire collects detailed information about the city's current life and disability insurance programs, including enrollment data, benefit structures, plan designs, and employee demographics. The document requests specific information regarding the number of employees eligible for coverage, current plan participants, benefit amounts, waiting periods, elimination periods, and any voluntary or supplemental coverage options currently offered. The questionnaire serves as a needs assessment tool to enable potential vendors to understand the scope and scale of the insurance services required and to tailor their proposals accordingly.

The questionnaire format indicates that responses are essential for evaluating vendor capabilities to service the City of Burleson's life and disability insurance needs. The information gathered through this questionnaire will inform the final contract award and pricing structure for the ancillary insurance benefits program. As a standardized data collection instrument, it establishes baseline requirements and expectations for all responding vendors, ensuring consistent evaluation criteria across all proposals submitted for the life and disability insurance components of the broader ancillary benefits contract opportunity.

Overview
We do not share renewals as part of our marketings. The renewal information will not be available for this RFP.
All questions must be asked through the City of Burleson's online bidding system, BonFire. Questions received outside of BonFire will not be responded to.
We suggest registering and logging in to BonFire early to avoid any technical issues. Bids received after the close date due to technical issues will not be accepted.
Do not submit rates that are dependent on the sale of VB/Worksite products such as Accident, Critical Illness or Hospital Indemnity unless instructed to do so.
Please answer all questions in the way they are intended (even if you are the incumbent). For example, if the question asks if you are including a provision/service/open enrollment, but there is an additional cost for you to include, please mark that it is not included and add the pricing in the notes.
Attached STD experience from UW , the other lines of coverage are not available due to the number of lives.
Please provide your most aggressive pricing upfront as we will not be having multiple rounds of negotiations; finalists will be determined off the initial submission so make it count. If you are selected as a finalist, there will be one round of pricing adjustments.
Client Requirements
Need a True Open enrollment (VL&VADD, VSTD)
+/-20% repricing threshold
RFP notes at least 2% in FI premium for implementation credits; would like to see $20,000 in implementation credits/tech credits (non-rate impacting)
