About this file
This document is an attachment to Request for Proposal BB27-103 issued by the State of Utah Division of Purchasing on behalf of the Department of Health and Human Services for a medication dispensing system contract. The RFP seeks a single vendor to provide parts, maintenance, support, and installation services for BD Pyxis Medication Dispensing System equipment, including MedStation units, auxiliary units, tower cabinets, and refrigerators with biometric access and dual-locking systems. The contract term is five years, with vendor pricing guaranteed for one year. The attachment establishes mandatory minimum requirements that vendors must satisfy to advance to technical evaluation, including the ability to provide and install specified BD Pyxis equipment, maintain all required licenses, deliver installation and maintenance services within defined response times (24 hours for critical failures, 3 business days for routine repairs, with troubleshooting initiated within 4 hours), and provide 24/7 qualified technical support via telephone, online access, remote diagnostics, and on-site service.
Vendors must also respond to four technical criteria that will be scored on a 0-5 scale by an evaluation committee: their approach to providing required equipment, their demonstrated ability to meet response time requirements, their capacity to deliver qualified personnel and comprehensive technical support, and their prior experience supporting BD Pyxis systems or similar automated medication dispensing systems in healthcare or correctional facilities. The procurement anticipates a single award to the responsive and responsible vendor achieving the highest combined score. No specific pricing terms, set-asides, incumbent vendor information, or funding amounts are detailed in this attachment.
