About this file
This is a cost proposal attachment for Solicitation MK27-1 issued by the State of Utah Division of Purchasing to provide accessibility products, installations, and maintenance services for patient mobility in homes and communities. The RFP encompasses five product categories: vehicle adaptive equipment, stairlifts, lifts, ramps, and walk-in showers, each with specified minimum equipment standards and installation requirements. Vendors may bid on one or more categories independently. The contract term is five years with a mandatory one-year price guarantee period. Vendors must achieve a minimum technical score of 600 points and a combined technical and cost score of at least 750 points to be awarded a contract. Multiple awards are anticipated.
The cost proposal uses a competitive scoring methodology whereby 333.3 cost points are available per category, with the vendor offering the lowest average cost receiving maximum points and other vendors receiving proportional points based on the formula: Cost Points × (Lowest Proposed Average County Price / Proposed Average County Price). Vendors must complete yellow-highlighted fields for categories they pursue, including product pricing or percentage discounts from MSRP, hourly installation rates, mileage rates, and maintenance/repair rates. The cost sheet calculates a total cost using a hypothetical scenario of purchasing all equipment items in each category, with 15 hours of installation labor, 50 miles of mileage, and 2 hours of maintenance labor per item. An administrative fee of 0.25 percent applies to the contract. Vendors offering modified used vehicles must demonstrate vehicle modification capability and provide percentage discounts off published pricing; this offering requires prior award of Category 1.

ATTACHMENT C (Cost Proposal) - for Solicitation #MK27-1
PURPOSE - This Attachment is used to evaluate and assign cost points to a vendor's proposal; however, not all requested information in this attachment will be used to assign cost points. All pricing information provided by the vendor will become contractual if the vendor is awarded a contract. Awarded vendors will be required to provide a full catalog or price list, which will be included in the contract. Any % discounts will be for the life of the contract and you may update your catalog as needed and pricing will be for the duration of the price guarantee period as stated in the RFP.
COST POINTS - Cost points available: 333.3 (per category). Each category is independent and scored separately. The vendor with the lowest average cost for all items in each category, will receive the maximum cost points for that category. All other vendors will receive a portion of the total cost points available based on a percentage of the lowest offered price. The formula to compute the points is: Cost Points x (Lowest Proposed Average County Price/Proposed Average County Price). Cost points will be added to a Vendor's Technical Score to determine a total RFP score.
