The contract compensation structure provides $500.00 per month per facility for a monthly total of $5,500.00, with monthly invoices submitted to designated DHHS contacts. The contractor must include standard billing documentation including name, payment address, phone number, contract and invoice numbers, and itemized service details with hours worked. Quarterly reports are required within 30 days after each calendar quarter end in formats specified by the DHHS designee. This contract is being awarded under a Notice of Intent to Award without engaging in standard competitive procurement, indicating the state has determined that either a single source exists for this procurement, transitional costs are significant, or other circumstances make standard procurement impractical and not in the best interest of the state.
