About this file
This is a vendor disclosure form from the Colorado Department of Corrections (CDOC) requiring bidders to disclose whether contract performance will occur outside the United States or the State of Colorado. The form must be completed and returned with all bids submitted in response to this solicitation. Bidders are required to answer whether any services under the contract or subcontracts will be performed outside the United States or Colorado, and if so, must provide details regarding the type of services, specific locations where services will be performed, and justification for performing work outside these geographic boundaries. The form requires completion and signature by an authorized representative such as the business president, board chairperson, or contract authorized signatory.
The form serves as formal notice to the Department of Personnel in compliance with Colorado Revised Statutes 24-102-206 and 24-102-205, establishing a mechanism for transparency regarding offshore or out-of-state contract performance. Procurement personnel must verify whether vendors provided required notice within 20 days of contracting or deciding to contract outside the United States or Colorado. This disclosure requirement applies to all non-construction contracts and ensures the state maintains visibility into vendor activities that may impact service delivery, compliance, and economic considerations related to state contracting decisions.
