About this file

This is a Campaign Contribution Disclosure Form issued by the Incorporated County of Los Alamos Procurement Division for prospective contractors bidding on the San Ildefonso 8" Waterline Replacement project in New Mexico. The form requires all prospective contractors to disclose whether they, a family member, or a representative have made campaign contributions totaling more than $250 in the aggregate to any applicable public official—specifically the seven named County Councilors (Theresa Cull, Melanee Hand, Susie Havemann, Ryn Hermann, Beverly Neal-Clinton, David Reagor, and Randal Ryti)—during the two-year period prior to proposal submission. The disclosure requirement applies to all prospective contractors regardless of whether contributions were made and must be completed for competitive sealed proposals as well as sole source and small purchase contracts.

The form defines key terms including "applicable public official," "campaign contribution," "family member," "representative of a prospective contractor," and establishes that the disclosure period covers the two years prior to contract signing for sole source or small purchase contracts. Prospective contractors must either confirm that contributions over $250 were made and disclose those contributions in detail, including the contributor's name, their relationship to the contractor, the official's name, contribution dates, amounts, and nature of contribution, or confirm that no contributions over $250 were made. The form must be signed and dated by an authorized representative, and additional pages may be attached if necessary to document multiple contributions.
