B.2 PERFORMANCE WORK STATEMENT (PWS)
LKNA Active Vehicle Barrier Onetime Repairs
I. Purpose:
The purpose of this Performance Work Statement (PWS) is to repair active vehicle barriers to normal operational status at The Orlando Veterans Affairs Medical Center (OVAMC) at Lake Nona located on 13800 Veterans Way, Orlando FL 32827.
II. Scope:
The Contractor shall provide all personnel, labor, equipment, supplies, tools, materials, supervision, and other items and services necessary to provide regular Full Repair Service for Nasatka Inc. popup barrier systems as specific, shall be performed on Magnetic Autocontrol Corp. drop arm barriers, Viking Access System drop arm barriers and all other gate control systems listed. Repairs shall be performed for all barriers at Orlando Veterans Administration Medical Center (Lake Nona Campus) as listed in Appendix A herein. Work shall comply with commercial standards for maintenance and repair of these units as well as all commercial, federal, state, and local requirements that may apply. All inspection reports shall be provided by the contractor within 7 (seven) days after completion of repairs. All barriers consist of but not limited to actuators, control wiring, circuitry, structural framework, lighting, spring assembly, hydraulic related equipment, corresponding traffic lights, associated computerized systems and control cabinets.

III. Period and Place of Performance:
This performance period is effective on 4 June 2018. All repairs that are not waiting on parts shall be completed within 90 days (180 days maximum for parts on order). The Place of Performance is at the Orlando VA Medical Center (Lake Nona Campus), located at 13800 Veterans Way, Orlando Florida 32827.
1.1 Normal workdays are Monday through Friday, 7:00am to 4:30pm, excluding Federal Holidays listed in below chart, and any other day specifically declared by the President of the United States to be a Federal Holiday.

IV. Background:
The Orlando VAMC needs repairs to all active vehicle barriers and gates to make the system fully operational.
V. Performance Objective (Required results):
The objective of this PWS is to allow for a proposed firm fixed price and solution to known requirements to repair of the vehicle barriers, drop arms and gates as stated in part II Scope, of this document. Offerors shall use the PWS, together with other applicable portion of this Request for Quote.
The contractor shall provide full inspection and repair in accordance with equipment manufacturer’s specifications/recommendations, and industry standard practices to include repairs on any computerized systems that support the active vehicle barrier. Contractor shall repair and replace all worn or failed components, installed additional systems (i.e. cameras, video processing cards, PLC’s, relays) and parts— including those parts which have reached their life expectancy and are prone to fail. Replacement parts shall be fully compatible with existing systems. Contractor shall provide trained mechanics and inspectors for all barrier maintenance and repair. All repair inspection reports once completed and the system is fully operational shall be provided to the Contracting Officer’s Representative (COR) within seven (7) working days after completion. Completion reports shall have the actual signature (nonphotocopies) of the onsite project manager/technician performing all the repairs.
1.1. REPAIRS, FULL PREVENTIVE MAINTENANCE INSPECTIONS (FPMI), AND SPECIAL MAINTENANCE PROJECTS (SMP): The contractor shall perform a onetime repair to ensure reliable and continuous safe operations of all equipment. Repair documents shall be submitted to the Contracting Officer Representative (COR) once all work has been completed. Contractor shall ensure barriers meet manufacturer maintenance performance specifications. The contractor shall determine crew size needed for repairs, or standard maintenance procedure (SMP)’s and payment will be based on one (1) time repair. Crew/on site supervisor shall notify the COR they have arrived on campus and will notify the COR when they are finished and leaving the campus. Payment for travel time is not authorized. Should the contractor require additional support from factory or other representatives these individuals shall be considered as part of the repair and not billed separately.
1.2. MAINTENANCE MANUALS, LOGS, and Repairs: The contractor shall obtain from the system manufactures all maintenance manuals and schematics prior to starting work with this contract. The contactor shall maintain repair data base with all part nomenclature, part numbers and serial numbers of all parts installed. The contractor shall update all manuals, schematics as changes occur and provide as built schematics/diagrams in the event system changes have occurred. The contactor shall develop a maintenance repair log to include a complete history of all maintenance and repairs, indicating dates and name of technician who performed the work. Copies of all maintenance logs shall be provided to the COR once contract is completed.
1.3. COORDINATION: The contractor shall coordinate all repairs with the COR and VA Police so repairs are conducted in a safe matter during the normal hours of operation and can adequately redirect traffic, if necessary. The contractor shall wear road safety reflective vest and provide all safety warning cones in accordance with Department of Transportation and Occupational Safety and Health Administration requirements.
1.4. BARRIER PIT: The contractor shall maintain the cleanliness of barrier pits and shafts, which shall be free of trash, debris, oil, absorbents, dirt, silt, pineneedles and water.
1.5. PAINT/APPEARANCE: Barrier systems shall be repaired with High Adhesion Paint such as GripTite® paint or an equivalent adhesive paint. At a minimum, they shall be completed during the onetime contract, sandblasted and repainted. The 5year requirement for sandblasting and repainting of the barriers shall be completed by the contractor for barrier repair under this SOW.
1.6. COMMUNICATION AND RESPONSE TIME: The contractor will provide the Government with at least one primary and one alternate emergency contact phone number to during the repair operations. The government shall coordinate repairs actions directly with the local onsite COR.
| 1.7. | ONE TIME REPAIR: Contractor shall perform an initial assessment of all systems and shall make all repairs, modifications, adjustments, calibrations and/or replacement of system components as required to restore all systems to a fully operational status. The following items have been identified by an independent barrier system specialist as requiring correction: |
| 1.8. | AUTHORIZED PERSONNEL: The Contracting Officer (CO) will provide the contractor a list of Contracting Officer Representatives (COR) authorized to request/accept services for the Government. The contractor will only accept work from designated COR. |
All repairs and conditions found, are listed below:

· Condition Found: traffic arms will not power up and arms had to be removed due to internal hardware failure and arms falling across traffic lanes Action: Since replacement parts are not available replace these with the same style of Traffic Arms in other locations: MFG: Magnetic Automation Model: Access PROLRC02040 Arms are 12’ each with breakaway arm option.
· Condition Found: Access control has been removed from Center Island.
Action: Install Enforcer Keypad on a gooseneck stand to match other keypads on campus. This will control the opening of entry traffic arm. Install a “free Exit loop” to control the opening of the exit arm Install a safety loop/closing loop on the road near each arm to close the arms and prevent vehicle strikes. This is a total of 4 loops one on either side of each traffic arm.

· Condition Found: NE Traffic Arm mount is loose and arm is twisted (Figure 2) Action: Adjust Traffic Arm housing so it is square with remaining arms and tighten Bolts. Replace bolts with new anchor bolts if necessary.

Main Entrance – North East Barriers MFG: Nasatka Security 3 each Wedge type Barriers Model # 111dDual S/N: 114595
· Condition Found: Hydraulic oil is extremely dirty.
Action: The oil, suction strainer needs to be replaced. The hydraulic drum needs to be wiped and cleaned thoroughly. This is an annual maintenance item to ensure the system remains healthy and internal parts do not degrade.
· Condition Found: Hydraulic hoses are expired with a date code of 2011 and hoses are to long.
Action: The hydraulic hoses need to be the replaced and made on site to exact lengths. As hoses get older they degrade and will blow if not changed. Hoses that are not routed properly and the proper length cause undue stress on the power unit and cause the hoses to wear out prematurely.
· Condition Found: In Main control panel. 22 each inline terminal block surge protectors are failing. Some are fused in direct short between upper and lower terminals causing the system to malfunction.
Action: Replace with whole unit surge protection, normal terminal blocks. (See pictures below)
· Condition Found: Barrier Front Rubber pad missing/damaged.
Action: Replace with OEM new rubber pads.
· Condition Found: Additional layers of system controls are unnecessary and overcomplicating the system.
Action: Remove additional layers of industrial controls and Reprogram master controls and streamline operations. The use of industry standard industrial controls design will yield a more stable system. This design is easier to troubleshoot and repair. This should be done reutilizing as much of the existing system as possible.
· Condition Found: Drains clogged standing water and debris in barrier pits and joints Action: Clear drains and clean residue from barrier pits. It is important to keep the barrier pits free of standing water and debris. If not maintained, over time this is one of the largest contributors to barrier decay. It also makes the barriers unsightly and creates a breeding ground for mosquitos and other insects that can harm the electronic components of the system.
· Condition Found: Failing leaking down valve Action: Replace with OEM or equivalent valve. This valve controls the motion from the Secure to the unsecure position. Besides making a mess and potentially expelling all of the systems oil, it will eventually fail and could cause unexpected motion of the barrier.
· Condition Found: Concrete pad under is poured to high and is causing damage to the barrier Action: Grind down concrete so it doesn’t contact the underside of the barrier. Touch up bottom barrier paint as needed. This will allow the barrier to close fully and rest on the designed supports. If not repaired this will not only cause cosmetic damage it will cause undue stress on a number of the barriers components causing premature failure.
· Condition Found: Concrete slurry is present on top of barrier plate.
Action: Removal is required and repaint the barrier Top
· Condition Found: Limits Sensors not working correctly and Limit wiring is not properly terminated for below grade application and in most cases wire is damaged in the conduit.
Action: Recommended to replace all lead wires and limit sensors. The wire should be a Water blocking 4 conductor limit lead. The cable should be shortened, terminations should be soldiered, adhesive heat shrunk, and cable sleeved. The limit sensors report the position of the barrier to the control system. Without them things like the lights traffic lights etc don’t work correctly. The terminations and wires should be water tight and protected against the moving parts within the barrier
· Condition Found: Noticeable rusting and paint chipping is on the outdoor HPU enclosure Action: The loose and chipping paint needs to be removed so the cabinet can be repainted with industrial outdoor paint. The enclosure is the Hydraulic Power Units defense from the harsh Florida elements and needs to remain intact.
· Condition Found: Safety Loop #1 and #3 tested bad Action: Replace loops, cut in two new loops, run new loop lead wire, and seal with traffic loop sealant. The safety loops report when an obstruction is near the barrier. If a loop fail the barrier can deploy even when a vehicle is present causing an unintended impact.
· Condition Found: Paint on barriers is damaged Action: Touch up underside of barrier and Repaint top sides. This is the barriers first line of defense against Florida’s harsh elements. If neglected they barriers will decay and fail prematurely.

VII. Clearances:
Contractor’s personnel shall have the credentials to enter OVAMC. In addition, the Government will provide assistant in obtaining installation and workplace access during the period of performance. All access identification will be returned to the government at the completion of each work day. Contractor (s) shall wear a contractor’s identification badge always while on Government property and shall be escorted by a VA employee.
VIII. Deliverables:
Deliverables will be established by the Performance Work Statement (PWS), Offeror’s quote, and subsequent fixed priced contract.
| 1. ORIGINAL EQUIPMENT MANUFACTURER (OEM) REPLACEMENT PARTS: All parts and materials shall be of OEM replacement parts or equal, recommended by the manufacturer. All parts determined to be equal will be approved by the COR prior to purchase. All parts shall be U.L. (Underwriters Laboratories) approved/listed. Only new standard parts shall be used in completed repairs. All replaced parts obtained by the contractor shall be invoiced at contractor’s cost. Additional cost/markup on materials of work performed by subcontractor will not be accepted. If applicable, contractors will abide by any existing active mechanical warranties. | ||
| 1.1. | SERVICE REPORTS: All parts and materials used service calls shall be itemized in the report and provided to the government. A signed report, by the onsite supervisor, shall be provided and include the date and time, location of barrier system, repairs performed, date and times repairs were completed, description of parts/materials replaced, actual cost, contract number, and name of the technician performing the repairs. | |
| 1.2. | BARRIER ASSESSMENT REPORT: The contractor shall prepare and submit a written report to the COR within seven (7) days after performance. The report shall include identification of each barrier system, location. repair performed, date of inspection, inspector, and overall condition of the barrier system. The contractor shall submit one report, sectioned with tabs according to barrier system location, to the designated COR. The contractor shall also submit now the repair log to include all work and parts replaced on all barriers for the onetime contract. This log shall include at a minimum: work performed, part nomenclature, part and serial numbers, and action taken. |
B.3 LIMITATIONS ON SUBCONTRACTING-- MONITORING AND COMPLIANCE (JUN 2011)
This solicitation includes FAR Clause 852.219-10, VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an “Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement” to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.

SECTION C - CONTRACT CLAUSES
C.1 52.212-4 CONTRACT TERMS AND CONDITIONS—COMMERCIAL ITEMS (JAN 2017)
(a) Inspection/Acceptance. The Contractor shall only tender for acceptance those items that conform to the requirements of this contract. The Government reserves the right to inspect or test any supplies or services that have been tendered for acceptance. The Government may require repair or replacement of nonconforming supplies or reperformance of nonconforming services at no increase in contract price. If repair/replacement or reperformance will not correct the defects or is not possible, the Government may seek an equitable price reduction or adequate consideration for acceptance of nonconforming supplies or services. The Government must exercise its post-acceptance rights—
(1) Within a reasonable time after the defect was discovered or should have been discovered; and
(2) Before any substantial change occurs in the condition of the item, unless the change is due to the defect in the item.
(b) Assignment. The Contractor or its assignee may assign its rights to receive payment due as a result of performance of this contract to a bank, trust company, or other financing institution, including any Federal lending agency in accordance with the Assignment of Claims Act (31 U.S.C. 3727). However, when a third party makes payment (e.g., use of the Governmentwide commercial purchase card), the Contractor may not assign its rights to receive payment under this contract.
(c) Changes. Changes in the terms and conditions of this contract may be made only by written agreement of the parties.
(d) Disputes. This contract is subject to 41 U.S.C. chapter 71, Contract Disputes. Failure of the parties to this contract to reach agreement on any request for equitable adjustment, claim, appeal or action arising under or relating to this contract shall be a dispute to be resolved in accordance with the clause at FAR 52.233-1, Disputes, which is incorporated herein by reference. The Contractor shall proceed diligently with performance of this contract, pending final resolution of any dispute arising under the contract.
(e) Definitions. The clause at FAR 52.202-1, Definitions, is incorporated herein by reference.
(f) Excusable delays. The Contractor shall be liable for default unless nonperformance is caused by an occurrence beyond the reasonable control of the Contractor and without its fault or negligence such as, acts of God or the public enemy, acts of the Government in either its sovereign or contractual capacity, fires, floods, epidemics, quarantine restrictions, strikes, unusually severe weather, and delays of common carriers. The Contractor shall notify the Contracting Officer in writing as soon as it is reasonably possible after the commencement of any excusable delay, setting forth the full particulars in connection therewith, shall remedy such occurrence with all reasonable dispatch, and shall promptly give written notice to the Contracting Officer of the cessation of such occurrence.
(g) Invoice.
(1) The Contractor shall submit an original invoice and three copies (or electronic invoice, if authorized) to the address designated in the contract to receive invoices. An invoice must include—
(i) Name and address of the Contractor;
(ii) Invoice date and number;
(iii) Contract number, line item number and, if applicable, the order number;
(iv) Description, quantity, unit of measure, unit price and extended price of the items delivered;
(v) Shipping number and date of shipment, including the bill of lading number and weight of shipment if shipped on Government bill of lading;
(vi) Terms of any discount for prompt payment offered;
(vii) Name and address of official to whom payment is to be sent;
(viii) Name, title, and phone number of person to notify in event of defective invoice; and
