About this file
This is a Contractor Agreement for Services document issued by Nebo School District, a political subdivision of the State of Utah located in Spanish Fork. The agreement establishes the terms and conditions for pest control services to be provided at forty-seven (47) District-owned facilities beginning in September 2026. The services include general exterior pest control provided three times per year. The contract has a five-year term from the date of execution, with provisions allowing the School District to terminate at any time with twenty-four (24) hours' written notice. The agreement requires that the contractor maintain proper licensing in the State of Utah and comply with all applicable federal, state, and local laws, regulations, building codes, environmental and safety standards, the Americans with Disabilities Act, and School District policies.
Compensation is calculated according to a Fee Schedule submitted by the contractor as Exhibit "B" to the agreement. The contractor may submit monthly progress billings, with the School District paying the proportionate amount of completed services as approved by the Operations Department and the remaining balance within ten (10) business working days of final completion. The contractor is classified as an independent contractor and is responsible for providing all necessary transportation, tools, supplies, and equipment at its own expense. The contractor must maintain comprehensive general liability insurance with minimum limits of $1,000,000 per occurrence and $3,000,000 in the aggregate, complete workers' compensation insurance, and automobile insurance for all vehicles used in performing services. The School District must be named as an additional insured on all applicable policies. The contractor assumes full responsibility for all taxes, worker's compensation coverage for its employees, and indemnification of the School District from claims arising from the contractor's negligent performance or acts. The contractor must also verify employee work eligibility status through E-Verify and certify that all personnel and subcontractors are not listed on the federal System for Award Management exclusion database.
