About this file

This document contains a Relevant Experience Questionnaire template for the Emergency Medical Kit Life Cycle Sustainment solicitation issued by the Department of State. The solicitation seeks proposals from firms with experience managing standardized emergency response kits and supplies, including assembly, distribution, warehousing, data management, tracking, and life cycle sustainment services for kits and supplies used at all Department of State posts worldwide. The template requires information on the offeror's point of contact and proposed team members, and details on up to six relevant past performance examples demonstrating experience with projects of similar size, scope, and complexity, including descriptions of the work performed, clients, and involvement of proposed team members. References in the form of CPAR reports or past performance questionnaires may be provided for each example.
