About this file
This document is a Vendor Work Order Request template used by the State of North Dakota for procuring vendor services. The form is designed to facilitate work order requests under existing state contracts, providing a standardized process for agencies to engage vendors for specific projects or services. The template (SFN 54383) outlines a comprehensive framework for defining project scope, staffing requirements, and vendor selection criteria.
The work order request allows for two primary agreement types: Time and Materials or Fixed Bid Project, with a structured evaluation process that allocates points across vendor project proposal (30 points), staff qualifications and availability (40 points), and cost (30 points). Vendors are required to submit detailed proposals including project strategy, staff resumes, reference checks, and pricing information. The document emphasizes vendor accountability through various reporting requirements such as daily accountability, weekly status reports, and monthly reports. Notably, the state reserves the right to conduct background checks on vendor personnel and reject individuals based on criminal history record checks.
