SECTION 00301 MEASUREMENT AND PAYMENT

PART 1 - GENERAL

1.1 DESCRIPTION

A. This Section specifies measurement and payment for the work of this contract.

1.2 GENERAL

A. The basis for measurement and payment for all work performed under this contract shall be in accordance with the Schedule of Items (Section B). In the event of a conflict Section B shall prevail over this Section (00301).

B. Payment will be made at the contract prices listed in the Schedule of Items.

1.3 INCIDENTAL WORK

A. Work to be performed which is not specifically called out to be measured and/ or paid for shall be incidental to other related items listed in the Schedule of Items. There will be no separate measurement and payment for any work not specifically listed in the Schedule of Items.

B. The summation of all bid items under the Schedule of Items shall equal all work required by the Drawings, Specifications and incidentals to complete a functioning job regardless of whether all the work needed is described in the collective bid items.

1.4 PRICES FOR COMPLETE ITEM

A. The prices listed in the Schedule of Items shall be payment in full for all labor, tools, equipment, materials, permits, etc., which are required to construct respective bid items according to the Contract Documents, including all work and materials for a fully functioning item.

NOTE: All contract documents shall be referred to for completion of items. Drawing sheets are referenced below.

1.1 Erosion & Sediment Control BMP’s for decommissioning of buildings and utilities must be completed in accordance with Project Drawings and Final Closure Plan.

1.2 SWPP & Erosion & Sediment Control BMP’s for decommissioning of the wastewater system components must be completed in accordance with Project Drawings and Final Closure Plan.

2.0 Utility Building demolition must be completed in accordance with Project Drawings (Sheet C203)

3.0 Utility Service Decommission must be completed in accordance with Project Drawings (Sheet C304 and C305)

4.0 Water/Wastewater Conveyance Demolition must be completed in accordance with Project Drawings (Sheets C304, C305, C401, C404, C405, C406, and C407)

5.0 Lagoon Demolition and Land Application must be completed in accordance with Project Drawings (Sheets C201, C301, C402 and C403) and references from the Final Closure Plan.

6.0 OPTION ITEM: Excavate & Dispose of Soil Below the Liner. Payment based on loose cubic yard truck measurements.

1.5 PARTIAL PAYMENT ON UNIT AND LUMP SUM ITEMS

A. Payment for unit bid items and lump sum bid items only partially completed at the end of monthly invoice periods will be based on a percentage of work completed as determined by the Contracting Officer.

SECTION 01100 SUMMARY

1.1 SUMMARY

A. Particular Project Requirements:

1. Existing site conditions and restrictions: There is no electricity, water, or wastewater at this site. The Dale Compound is currently not occupied with exception to 2 buildings that are used as storage with seldom to no access by employees.

2. Prior hazardous waste or asbestos work by the Owner or others: The only asbestos work completed at this site is the survey and testing report that is on file with the Forest. Lead based paint is present on buildings at this site including the Dale Office Building.

B. Permits and Fees: Apply for, obtain, and pay for permits, fees, and utility company back-charges required to perform the work. Submit copies to the CO.

C. Codes: Comply with applicable codes and regulations of authorities having jurisdiction. Submit copies of inspection reports, notices and similar communications to the CO.

D. Dimensions: Verify dimensions indicated on the drawings with field dimensions.

E. Existing Conditions: Notify the COR of existing conditions differing from those indicated in the description of work. Do not remove or alter structural components without prior written approval.

F. Definitions:

1. Provide: Furnish and install, complete with all necessary accessories, ready for intended use. Pay for all related costs.

2. Approved: Acceptance of item submitted for approval. Not a limitation or release for compliance with the Contract Documents or regulatory requirements. Refer to limitations of “Approved” in General and Supplementary Conditions.

3. Match Existing: Match existing as acceptable to the Government.

G. Intent: Drawings and specifications are intended to provide the basis for proper completion of the work suitable for the intended use of the Government. Anything not expressly set forth but which is reasonably implied or necessary for proper performance of the project shall be included.

H. Writing style: Specifications are written in the imperative mode. Except where specifically intended otherwise, the subject of all imperative statements is the Contractor. For example, “Provide tile” means “Contractor shall provide tile.”

SECTION 024125 SELECTIVE DEMOLITION

1.1 RELATED DOCUMENTS

A. Drawings and General Provisions of the Contract, including General and Supplementary Conditions and other Division 01 Specification Sections, apply to this Section.

1.2 SUMMARY

A. This Section includes the following:

1. Demolition and removal of selected portions of building or structure.

2. Salvage of existing items to be reused or recycled.

1.3 DEFINITIONS

A. Remove: Detach items from existing construction and legally dispose of them off-site, unless indicated to be removed and salvaged or removed and reinstalled.

B. Existing to Remain: Existing items of construction that are not to be removed and that are not otherwise indicated to be removed, removed and salvaged, or removed and reinstalled.

1.4 MATERIALS GOVERNMENTSHIP

A. Historic items, relics, and similar objects including, but not limited to, cornerstones and their contents, commemorative plaques and tablets, antiques, and other items of interest or value to Government that may be encountered during selective demolition remain Government’s property.

1. Coordinate with Government’s Archaeologist, who will establish special procedures for removal and salvage.

1.5 SUBMITTALS

A. Qualification Data: For (asbestos removal certification)

B. Schedule below may be used to track Contractor’s progress; it may also be used to determine that selective demolition will not interfere with Government’s operations.

C. Schedule of Selective Demolition Activities: Indicate the following:

1. Detailed sequence of selective demolition and removal work, with starting and ending dates for each activity.

2. Locations of proposed dust- and noise- control temporary partitions and means of egress.

3. Means of protection for items to remain and items in path of waste removal from building.

D. Landfill Records: Indicate receipt and acceptance of hazardous wastes by a landfill facility licensed to accept hazardous wastes.

1.6 QUALITY ASSURANCE

A. Demolition Firm Qualifications: An experienced firm that has specialized in demolition work similar in material and extent to that indicated for this Project.

B. Regulatory Requirements: Comply with governing EPA and the State of Oregon notification regulations before beginning selective demolition. Comply with hauling and disposal regulations or authorities having jurisdiction.

C. Standards: Comply with ANSI A10.6, NFPA 214, ORS Chapter 466 – Hazardous Waste and Hazardous Materials II (Sections 466.706 to 466.845, 466.994 and 466.996), and OAR 340-248- 0010 through 340-248-0290.

1.7 PROJECT CONDITIONS

A. Comply with requirements specified in Division 01 Section “Summary”.

B. Conditions existing at time of inspection for bidding purpose will be maintained by Government as far as practical.

C. Notify Contracting Officer’s Representative of discrepancies between existing conditions and Drawings or specifications before proceeding with selective demolition.

D. Hazardous Materials: Hazardous materials are present in construction to be selectively demolished. A report on the presence of hazardous materials is attached.

E. Storage or sale of removed items or materials on-site is not permitted.

F. Utility Service. Maintain existing utilities indicated to remain in service and protect them against damage during selective demolition operations.

PART 2 - EXECUTION

2.1 EXAMINATION

A. Survey existing conditions and correlate with requirements indicated to determine extent of selective demolition required.

B. Inventory and record the condition of items to be removed and reinstalled and items to be removed and salvaged.

C. When unanticipated mechanical, electrical, or structural elements that conflict with intended function or design are encountered, investigate and measure the nature and extent of conflict.

Promptly submit a written report to Contracting Officer’s Representative.

D. Survey of Existing Conditions: Record existing conditions by use of measured drawings and photographs.

E. Perform surveys as the Work progresses to detect hazards resulting from selective demolition activities.

2.2 PREPARATION

A. Site Access and Temporary Controls: Conduct selective demolition and debris-removal operations to ensure minimum interference with roads, walks, walkways, and other adjacent facilities.

B. Temporary Facilities: Provide temporary barricades and other protection required to prevent injury to people.

1. Provide protection to ensure safe passage of people (contractor’s employees, and/or Government employees) around selective demolition area.

2. Protect walls, ceilings, floors, and other existing finish work that are to remain or that are exposed during selective demolition operations.

3. Cover and protect furniture, furnishings, and equipment that have not been removed.

2.3 SELECTIVE DEMOLITION, GENERAL

A. General: Demolish and remove existing construction only to the extent required and as indicated. Use methods required to complete the Work within limitations of governing regulations and as follows:

1. Neatly cut openings and holes plumb, square, and true to dimensions required. Use cutting methods least likely to damage construction to remain or adjoining construction.

2. Use hand tools or small power tools designed for sawing, not hammering and chopping, to minimize disturbance of adjacent surfaces. Temporarily cover openings to remain.

3. Cut or drill from the exposed or finished side into concealed surfaces to avoid marring existing finished surfaces.

4. Locate selective demolition equipment and remove debris and materials so as not to impose excessive loads on supporting walls, floors, or framing.

5. Dispose of demolished items and materials promptly.

B. Existing Items to Remain: Protect construction indicated to remain against damage and soiling during selective demolition. When permitted by Contracting Officer’s Representative items may be removed to a suitable, protected storage location during selective demolition and cleaned, and reinstalled in their original locations after selective demolition operations are complete.

2.4 SELECTIVE DEMOLITION PROCEDURES FOR SPECIFIC MATERIALS

A. Resilient Floor Coverings: Remove floor coverings and adhesive according to recommendations in RFCI-WP and its Addendum. Remove residual adhesive.

2.5 DISPOSAL OF DEMOLISHED MATERIALS

A. General: Except for items or materials indicated to be reused, salvaged, reinstalled, or otherwise indicated to remain Government’s property, remove demolished materials from Project site and legally dispose of them in an EPS-approved landfill.

1. Do not allow demolished materials to accumulate on-site.

2. Remove and transport debris in a manner that will prevent spillage on adjacent surfaces and areas.

3. Burning: Do not burn demolished materials.

4. Disposal: Transport demolished materials off Government’s property and legally dispose of them at a certified, appropriate, disposal facility.

2.6 CLEANING

A. Clean adjacent structures and improvements of dust, dirt, and debris caused by selective demolition operations. Return adjacent areas to condition existing before selective demolition operations began.

SECTION 312000 - EARTH MOVING

1.1 SUMMARY

A. Section Includes:

1. Excavating and filling for rough grading the Site.

2. Excavating and backfilling for buildings and structures.

3. Excavating and backfilling trenches for utilities and pits for buried utility structures.

1.2 DEFINITIONS

A. Backfill: Soil material used to fill an excavation. Reference Project Drawings for Class A, Class B and Class E backfill descriptions.

1. Initial Backfill: Backfill placed beside and over pipe in a trench, including haunches to support sides of pipe.

2. Final Backfill: Backfill placed over initial backfill to fill a trench.

B. Base Course: Aggregate layer placed between the subbase course and hot-mix asphalt paving.

C. Bedding Course: Aggregate layer placed over the excavated subgrade in a trench before laying pipe.

D. Borrow Soil: Satisfactory soil imported from off-site for use as fill or backfill.

E. Drainage Course: Aggregate layer supporting the slab-on-grade that also minimizes upward capillary flow of pore water.

F. Excavation: Removal of material encountered above subgrade elevations and to lines and dimensions indicated.

1. Authorized Additional Excavation: Excavation below subgrade elevations or beyond indicated lines and dimensions as directed by Architect. Authorized additional excavation and replacement material will be paid for according to Contract provisions for changes in the Work.

2. Unauthorized Excavation: Excavation below subgrade elevations or beyond indicated lines and dimensions without direction by Architect. Unauthorized excavation, as well as remedial work directed by Architect, shall be without additional compensation.

G. Fill: Soil materials used to raise existing grades.

H. Structures: Buildings, footings, foundations, retaining walls, slabs, tanks, curbs, mechanical and electrical appurtenances, or other man-made stationary features constructed above or below the ground surface.

I. Subbase Course: Aggregate layer placed between the subgrade and base course for hot-mix asphalt pavement, or aggregate layer placed between the subgrade and a cement concrete pavement or a cement concrete or hot-mix asphalt walk.

J. Subgrade: Uppermost surface of an excavation or the top surface of a fill or backfill immediately below subbase, drainage fill, drainage course, or topsoil materials.

K. Utilities: On-site underground pipes, conduits, ducts, and cables as well as underground services within buildings.

1.3 INFORMATIONAL SUBMITTALS

A. N/A

1.4 FIELD CONDITIONS

A. Utility Locator Service: Notify utility locator service for area where Project is located before beginning earth-moving operations.

B. Do not commence earth-moving operations until plant-protection measures specified in Project Drawings.

PART 2 - PRODUCTS

2.1 SOIL MATERIALS

A. General: Provide borrow soil materials when sufficient satisfactory soil materials are not available from excavations.

B. Materials availability:

1. Class A material available onsite and at nearby location (see Map attached)

2. Class B material not available onsite and must be delivered onsite

2.2 ACCESSORIES

A. Warning Tape: Acid- and alkali-resistant, polyethylene film warning tape manufactured for marking and identifying underground utilities, 6 inches (150 mm) wide and 4 mils (0.1 mm) thick, continuously inscribed with a description of the utility; colored to comply with local practice or requirements of authorities having jurisdiction.

B. Detectable Warning Tape: Acid- and alkali-resistant, polyethylene film warning tape manufactured for marking and identifying underground utilities, a minimum of 6 inches (150

mm) wide and 4 mils (0.1 mm) thick, continuously inscribed with a description of the utility, with

12 | P a g e metallic core encased in a protective jacket for corrosion protection, detectable by metal detector when tape is buried up to 30 inches (750 mm) deep; colored to comply with local practice or requirements of authorities having jurisdiction.

PART 3 - EXECUTION

3.1 PREPARATION

A. Protect structures, utilities, sidewalks, pavements, and other facilities from damage caused by settlement, lateral movement, undermining, washout, and other hazards created by earth-moving operations.

B. Protect and maintain erosion and sedimentation controls during earth-moving operations.

C. Protect subgrades and foundation soils from freezing temperatures and frost. Remove temporary protection before placing subsequent materials.

3.2 EXCAVATION, GENERAL

A. Unclassified Excavation: Excavate to subgrade elevations regardless of the character of surface and subsurface conditions encountered. Unclassified excavated materials may include rock, soil materials, and obstructions. No changes in the Contract Sum or the Contract Time will be authorized for rock excavation or removal of obstructions.

1. If excavated materials intended for fill and backfill include unsatisfactory soil materials and rock, replace with satisfactory soil materials.

3.3 EXCAVATION FOR STRUCTURES

A. Excavate to indicated elevations and dimensions within a tolerance of plus or minus 1 inch (25 mm). If applicable, extend excavations a sufficient distance from structures for placing and removing concrete formwork, for installing services and other construction, and for inspections.

1. Excavation for Underground Tanks, Basins, and Mechanical or Electrical Utility Structures:

Excavate to elevations and dimensions indicated within a tolerance of plus or minus 1 inch (25 mm). Do not disturb bottom of excavations intended as bearing surfaces.

B. Excavations at Edges of Tree- and Plant-Protection Zones:

1. Excavate by hand or with an air spade to indicated lines, cross sections, elevations, and subgrades. If excavating by hand, use narrow-tine spading forks to comb soil and expose roots. Do not break, tear, or chop exposed roots. Do not use mechanical equipment that rips, tears, or pulls roots.

2. Cut and protect roots according to requirements in Section 015639 "Temporary Tree and Plant Protection."

3.4 EXCAVATION FOR WALKS AND PAVEMENTS

A. Excavate surfaces under walks and pavements to indicated lines, cross sections, elevations, and subgrades.

3.5 EXCAVATION FOR UTILITY TRENCHES

A. Excavate trenches to indicated gradients, lines, depths, and elevations.

B. Excavate trenches to uniform widths to provide the following clearance on each side of pipe or conduit. Excavate trench walls vertically from trench bottom to 12 inches (300 mm) higher than top of pipe or conduit unless otherwise indicated.

1. Clearance: [12 inches (300 mm) each side of pipe or conduit] [As indicated].

C. Trench Bottoms: Excavate and shape trench bottoms to provide uniform bearing and support of pipes and conduit. Shape subgrade to provide continuous support for bells, joints, and barrels of pipes and for joints, fittings, and bodies of conduits. Remove projecting stones and sharp objects along trench subgrade.

1. Excavate trenches 6 inches (150 mm) deeper than elevation required in rock or other unyielding bearing material to allow for bedding course.

D. Trenches in Tree- and Plant-Protection Zones:

1. Hand-excavate to indicated lines, cross sections, elevations, and subgrades. Use narrow-tine spading forks to comb soil and expose roots. Do not break, tear, or chop exposed roots. Do not use mechanical equipment that rips, tears, or pulls roots.

2. Do not cut main lateral roots or taproots; cut only smaller roots that interfere with installation of utilities.

3. Cut and protect roots according to requirements in Section 015639 "Temporary Tree and Plant Protection."

3.6 SUBGRADE INSPECTION

A. Proof-roll subgrade with a pneumatic-tired dump truck to identify soft pockets and areas of excess yielding. Do not proof-roll wet or saturated subgrades.

B. Reconstruct subgrades damaged by freezing temperatures, frost, rain, accumulated water, or construction activities, as directed by Architect, without additional compensation.

3.7 STORAGE OF SOIL MATERIALS

A. Stockpile borrow soil materials and excavated satisfactory soil materials without intermixing.

Place, grade, and shape stockpiles to drain surface water. Cover to prevent windblown dust.

1. Stockpile soil materials away from edge of excavations. Do not store within drip line of remaining trees.

3.8 UTILITY TRENCH BACKFILL

A. Place backfill on subgrades free of mud, frost, snow, or ice.

B. Place and compact bedding course on trench bottoms and where indicated. Shape bedding course to provide continuous support for bells, joints, and barrels of pipes and for joints, fittings, and bodies of conduits.

C. Initial Backfill: Place and compact initial backfill of [subbase material] [satisfactory soil], free of particles larger than [1 inch (25 mm)] in any dimension, to a height of 12 inches (300 mm) over the pipe or conduit.

1. Carefully compact initial backfill under pipe haunches and compact evenly up on both sides and along the full length of piping or conduit to avoid damage or displacement of piping or conduit. Coordinate backfilling with utilities testing.

D. Final Backfill: Place and compact final backfill of satisfactory soil to final subgrade elevation.

E. Warning Tape: Install warning tape directly above utilities, 12 inches (300 mm) below finished grade, except 6 inches (150 mm) below subgrade under pavements and slabs.

3.9 SOIL FILL

A. Plow, scarify, bench, or break up sloped surfaces steeper than 1 vertical to 4 horizontal so fill material will bond with existing material.

B. Place and compact fill material in layers to required elevations as follows:

1. Under grass and planted areas, use satisfactory soil material.

2. Under walks and pavements, use satisfactory soil material.

3. Under steps and ramps, use engineered fill.

4. Under building slabs, use engineered fill.

5. Under footings and foundations, use engineered fill.

3.10 SOIL MOISTURE CONTROL

A. Uniformly moisten or aerate subgrade and each subsequent fill or backfill soil layer before compaction to within 2 percent of optimum moisture content.

1. Do not place backfill or fill soil material on surfaces that are muddy, frozen, or contain frost or ice.

2. Remove and replace, or scarify and air dry, otherwise satisfactory soil material that exceeds optimum moisture content by 2 percent and is too wet to compact to specified dry unit weight.

3.11 COMPACTION OF SOIL BACKFILLS AND FILLS

A. Place backfill and fill soil materials in layers not more than [8 inches (200 mm)] in loose depth for material compacted by heavy compaction equipment and not more than 4 inches (100 mm) in loose depth for material compacted by hand-operated tampers.

B. Place backfill and fill soil materials evenly on all sides of structures to required elevations and uniformly along the full length of each structure.

C. Compact soil materials to not less than the following percentages of maximum dry unit weight according to [ASTM D698] [ASTM D1557]:

3.12 GRADING

A. General: Uniformly grade areas to a smooth surface, free of irregular surface changes. Comply with compaction requirements and grade to cross sections, lines, and elevations indicated.

B. Site Rough Grading: Slope grades to direct water away from buildings and to prevent ponding.

Finish subgrades to elevations required to achieve indicated finish elevations, within the following subgrade tolerances:

1. Turf or Unpaved Areas: Plus or minus [1 inch (25 mm)]

2. Walks: Plus or minus [1 inch (25 mm)]

3. Pavements: Plus or minus [1/2 inch (13 mm)]

3.13 SUBBASE AND BASE COURSES UNDER PAVEMENTS AND WALKS

A. Place subbase course[ and base course] on subgrades free of mud, frost, snow, or ice.

B. On prepared subgrade, place subbase course[ and base course] under pavements and walks as follows:

1. Shape subbase course[ and base course] to required crown elevations and cross-slope grades.

2. Place subbase course[ and base course] that exceeds 6 inches (150 mm) in compacted thickness in layers of equal thickness, with no compacted layer more than 6 inches (150

mm) thick or less than 3 inches (75 mm) thick.

3. Compact subbase course[ and base course] at optimum moisture content to required grades, lines, cross sections, and thickness to not less than [95] <Insert number> percent of maximum dry unit weight according to [ASTM D698] [ASTM D1557].

3.14 FIELD QUALITY CONTROL

A. Special Inspections: Owner will engage a qualified special inspector to perform inspections:

B. Testing Agency: Owner will engage a qualified geotechnical engineering testing agency to perform tests and inspections.

C. Allow testing agency to inspect and test subgrades and each fill or backfill layer. Proceed with subsequent earth moving only after test results for previously completed work comply with requirements.

D. When testing agency reports that subgrades, fills, or backfills have not achieved degree of compaction specified, scarify and moisten or aerate, or remove and replace soil materials to depth required; recompact and retest until specified compaction is obtained.

3.15 PROTECTION

A. Protecting Graded Areas: Protect newly graded areas from traffic, freezing, and erosion. Keep free of trash and debris.

B. Repair and reestablish grades to specified tolerances where completed or partially completed surfaces become eroded, rutted, settled, or where they lose compaction due to subsequent construction operations or weather conditions.

SECTION 312500 SEDIMENT AND EROSION CONTROL MEASURES

1.1 SUMMARY

A. The work under this section consists of furnishing all necessary labor, equipment, materials, and performing all operations in connection with construction sediment and control measures.

B. General

1. All erosion and sediment control measures are to be placed prior to any disturbance caused by grading and or excavation and shall conform to the requirements of the appropriate regulatory agency for the State.

2. The Contractor shall be solely responsible for ensuring that erosion and sediment control measures are implemented and maintained at the site.

3. Soil disturbing activities include but are not limited to: Clearing and grubbing, excavation for utilities and foundations, construction or modification of site drainage, and grading.

PART 3 - EXECUTION

3.1 CONSTRUCTION

A. Install straw bales at local drainage ways to prevent silt intrusion upon adjacent drainage courses. Remove straw bales following establishment of vegetation cover and utilize as mulch at swales or on steep slopes.

B. Prior to construction, install silt fence along the downhill construction limits in accordance with the erosion control standard detail to prevent silt intrusion upon adjacent land.

C. Install sediment and erosion control measures on the down slope toe of all top soil stock piles.

D. Maintain and remove all erosion controls as specified.

3.2 DUST CONTROL

A. In areas subject to surface and air movement of dust, where on-site or off-site damage is likely to occur, one or more of the following preventive measures shall be taken for dust control:

1. Sprinkle the site with water until surface is wet. Repeat as needed.

3.3 SILT FENCE

A. Silt fences are appropriate for the following general locations:

1. Immediately upstream of the point(s) of runoff discharge from a site before flow becomes concentrated. Below disturbed areas where runoff may occur in the form of overland flow.

2. Along the down slope toe of all top soil stock piles.

B. Materials.

1. Utilize standard strength synthetic filter fabric for sediment barriers. The filter fabric shall be purchased in a continuous roll cut to the length of the barrier to avoid the use of joints. When joints are necessary, filter cloth shall be spliced together only at a support post, with a minimum 6 inch overlap, and securely sealed.

2. The standard strength filter fabric shall be stapled or wired to the fence and 6 inches of the fabric shall be extended into the ground. Filter fabric shall not be stapled to existing trees.

3. Support posts shall be spaced at a maximum 6 feet and driven securely into the ground a minimum of 24 inches.

4. The height of a silt fence shall not exceed 36 inches. Higher fences may impound volumes of water sufficient to cause failure of the structure.

C. Maintenance.

1. Silt fences and filter barriers shall be inspected immediately after each rainfall and at least daily during prolonged rainfall.

2. Silt fences shall be inspected for depth of sediment, tears, and to see if the fabric is securely attached to the fence posts, and to see that the fence posts are firmly in the ground. Any deficiencies shall be repaired immediately.

3. Should the fabric on a silt fence of filter barrier decompose or become ineffective prior to the end of the expected usable life and the barrier still be necessary, the fabric shall be replaced promptly.

4. Sediment deposits should be removed after each storm event and/or when deposits reach approximately 1/3 the height of the barrier or when the sediments limit or prevent the flow of water through the fabric hydraulic.

5. Any sediment deposits remaining in place after the silt fence or filter barrier is no longer required shall be dressed to conform with the existing grade, prepared, and seeded.

3.4 STRAW BALE EROSION CONTROL FENCE

A. Straw bale erosion control fences are appropriate for the following general locations:

1. Sheet flow applications: Straw bales shall be placed in a single row, lengthwise on the contour with ends of adjacent bales tightly abutting one another.

2. Channel flow applications: Straw bales shall be placed in a single row, lengthwise and oriented perpendicular to the direction of flow with ends of adjacent bales tightly abutting one another. The barrier shall be extended to such a length that the bottoms of the end bales are higher in elevation than the top of the lowest middle bale to assure that sediment laden runoff will flow either through or over the barrier but not around it.

B. The barrier shall be entrenched and backfilled. A trench shall be excavated the width of a bale and the length of the proposed barrier to a minimum depth of 4 inches. After the bales are staked and chinked, the excavated soil shall be backfilled against the barrier. Backfill shall conform to the ground level of the downhill side and shall be built up to 4 inches against the uphill side of the barrier.

C. Each bale shall be securely anchored by at least 2 stakes or rebar driven through the bale. The first stake in each bale shall be driven toward the previously laid bale to force the bales together. Stakes or rebar shall be driven a minimum of 12 inches into the ground or deep enough into the ground to securely anchor the bales, whichever is greater.

D. The gaps between bales shall be chinked (filled by wedging with straw to prevent water from escaping between the bales). Loose straw scattered over the area immediately uphill from a straw bale barrier tends to increase barrier efficiency.

3.5 MAINTENANCE.

A. Inspection shall be frequent and repair or replacement shall be made promptly as needed.

B. Straw bale carriers shall be removed when they have served their usefulness, but not before the upslope areas have been permanently stabilized.

1.04 Submittals

A. Plan of Action.

1. The Contractor shall submit a detailed plan of the procedures they intend to use to comply with the regulations included in this specification. The plan shall include the location and layout of decontamination areas, the sequencing of asbestos work, the interface of trades involved in the performance of work, methods to be used to assure the safety of building occupants and visitors to the site, disposal plan including location of approved disposal site, sampling and testing, and a detailed description of the methods to be employed to control pollution. The plan must be approved by the Contracting Officer prior to commencement of work for conformance to these specifications.

B. Contractor Qualification Information.

1. Record of experience qualifying the firm as a specialist in asbestos abatement shall be submitted with the bid. As a minimum, this shall include documentation of successful performance in asbestos removal including the name and address of the purchaser of the service, location of work performed, and a record of air monitoring as required by 29 CFR 1910.1001.

C. Laboratory Qualification Information.

1. Submit proof of qualifications of testing laboratory and personnel. Accreditation by the American Industrial Hygiene Association (AIHA) for asbestos work and

23 | P a g e certification that persons counting the samples have been judged proficient by successful participation in the National Institute for Occupational Safety and Health (NIOSH) Proficiency Analytical Testing (PAT) Program shall be considered sufficient proof of compliance. Submittal must be approved by the Contracting Officer prior to beginning any testing.

D. Certificates of Compliance.

1. Submit certification that vacuums, ventilation, and other equipment required to contain airborne asbestos fibers conform to Federal, State and Local regulations.

1.05 Quality Assurance

A. Contractor Qualifications.

1. The Contractor shall be a firm of established reputation (or, if newly organized, whose personnel have previously established a reputation in the field) that is regularly engaged in and that maintain a regular force of workers skilled in asbestos abatement and shall have performed this work on previous projects.

2. The Contractor and their employees shall be licensed to remove asbestos material as required by the State.

1.06 Regulatory requirements

A. The Contractor shall be responsible for obtaining approval for a waste disposal site in compliance with Section 61.25 of the EPA regulations.

B. The Contractor shall assume full responsibility and liability for compliance with all applicable Federal, State, and Local regulations pertaining to the protection of workers, visitors to the site, and persons occupying areas adjacent to the site. The Contractor is responsible for providing medical examinations and maintaining medical records of personnel as required by the applicable Federal, State, and Local regulations.

C. Asbestos Control Limits.

1. Inside Asbestos Work Area.

a. Air concentrations of asbestos shall not exceed an 8 hour time weighted average of 0.2 fibers (longer than 5 micrometers) per cubic centimeter of air.

See 29 CFR 1910.1001.

2. Outside Asbestos Work Area.

a. Air concentrations of asbestos shall be maintained at an 8 hour time weighted average below 0.1 fiber (longer than 5 micrometers) per cubic centimeter of air. This applies to all areas in the building while work is in progress, except for the asbestos work area.

3. Final Clearance.

a. After final clean up, concentrations shall be below 0.01 fibers per cubic centimeters of air.

1.07 Project Conditions

A. Abatement of asbestos containing materials in the Utility building prior to demolition.

B. Means of Egress.

1. Establish and maintain emergency and fire exits from the work area.

C. Use of Existing Facilities.

1. The Contractor may use electricity, water and sanitation facilities in the building for project use only without charge.

D. Access to Work Area.

1. Access to work area shall be through decontamination areas. The following shall have access to work areas:

a. Personnel properly trained and fully equipped for safety will be permitted in the work areas.

b. Qualified State and EPA personnel will be permitted in the work areas.

c. Contracting Officer and designated representatives will be permitted in the work areas.

2. The Contractor shall provide protective clothing and respirators for the above listed personnel.

2. Part 2 - PRODUCTS

2.01 Equipment

A. Protective Equipment.

1. Equipment, including protective clothing and respirators, used in the execution of this Contract and provided to visitors to the site shall comply with the applicable Federal, State, and local regulations.

B. Disposal Containers.

1. Plastic bags shall be "leak-tight,” 6 mil polyethylene. Bags may be placed in 55 gallon drums for additional protection. Bags and drums (if used) must be labeled as specified by NESHAPS or OSHA.

2.02 Materials

A. Wetting Agent.

1. The wetting agent shall be 50 percent polyoxethylene ester and 50 percent polyoxethylene ether, in concentration of one (1) ounce in five (5) gallons of water.

3. Part 3 - EXECUTION

3.01 Preparation

A. Isolate the work area for the duration of the work according to acceptable trade practice and code requirements.

B. Build double barriers of plastic sheeting at all entrances and exits to the work area so that the work area is always closed off by 1 barrier when workers enter or exit.

C. Cover all floor, ceiling, and wall surfaces in the work area with 6 mil plastic sheeting taped securely in place to protect from damages.

D. Before the work is begun, clean all removable items and equipment. Remove them from the work area and store as directed.

E. Cover all nonremovable items and equipment in the work area with plastic sheeting taped securely in place.

F. Remove all heating, ventilation and air-conditioning system filters; pack them in sealable plastic bags (6 mil minimum) for disposal in the approved waste disposal site.

G. Post warning signs and labels as required by 29 CFR 1910.1001 and ASTM E849 and as directed by the Contracting Officer.

3.02 Monitoring

A. Monitoring of airborne concentrations of asbestos shall be in accordance with 29 CFR

1910.1001. Monitor the airborne concentration of asbestos before starting work to obtain a base line fiber concentration in the affected areas. Then monitor once every 4 hours during the work shift inside the asbestos work area, outside the entrance to the asbestos work area, and at the exhaust opening of the local exhaust system. If monitoring shows airborne concentrations causing excessive levels, notify the Contracting Officer immediately. In addition, monitor the airborne concentrations of asbestos after final cleanup and removal of the enclosure of the asbestos control area.

3.03 Site Inspection

A. While performing asbestos abatement work, the Contractor shall be subject to on-site inspection by agency officials. Work shall also be subject to inspection by OSHA and EPA inspector (and/or local building or health officials).

3.04 Demolition Procedure

A. The asbestos material shall be sprayed with water containing a wetting agent in the form of a fine spray. The material shall be sufficiently saturated to prevent emission of airborne fibers in excess of the exposure limits.

B. The asbestos material shall be removed in small sections. Before beginning the next section, the material shall be packed while still wet into sealable plastic bags and placed into drums for transporting.

C. Asbestos material around ducts may be removed using "glove bags."

D. All plastic sheeting, tape, cleaning material, clothing and all other disposable material or items used in the work area shall be packed into sealable plastic bags, placed into drums and marked per regulations.

3.05 Cleaning

A. The Contractor shall clean all surfaces in the work area.

B. After 24 hours, the Contractor shall wet-clean all surfaces in the work area again.

C. After completion of the second cleaning operation, the Contractor and COR shall perform a complete visual inspection of the work area to ensure that the area is dust free. If dust is observed, further cleaning will be required at Contractor’s expense.

D. Monitor for airborne asbestos after final clean up, as described.

E. After all work areas have been found to be in compliance, all entrances and exits are to be unsealed and all plastic sheeting, tape and all other trash and debris are to be disposed of in sealable plastic bags (6 mil minimum) and disposed of with the other waste.

3.06 Disposal

A. Waste asbestos material shall be disposed of in accordance with all Federal and State regulations implementing the intent of the Resource Conservation and Recovery Act (40 CFR 260-265) and at an approved sanitary landfill. The "small quantity exclusion" of the regulations shall not apply to disposal of waste asbestos materials. Establish a temporary holding area approved by the COR for properly packaged asbestos waste.
