Project Name and Location: Dale Water-Wastewater System Decommission This amendment addresses the following (see attached addendum)

By signature in Block 15B Contractor releases the Government from all claims relating to this modification. This settlement is complete accord and satisfaction. Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

Replace page 7: Site Visit (a) An organized site visit has been scheduled for: April 23, 2020 at 10:00am PST.

Pre-Bid Inquiries (PBI’s) for Contract Bidding

PBI-001

Question: Can contractors use the existing raw water intake and supply lines to deliver water to the lagoons for land application process?

Answer: No, sequencing of work shall follow closure plan and drawings in the contract package.

PBI-002

Question: Can contractors dispose of concrete from demolition phase onsite?

Answer: No, concrete must be hauled off site and disposed of properly.

PBI-003

Question: Can contractors utilize lagoon liner cover as fill material for the lagoons? Or can this be land applied in accordance with

DEQ?

Answer: No, liner cover material shall be hauled off and disposed of appropriately. No alternate land application site is available to land apply the cover material.

PBI-004

Question: Can contractors have the contact information for DEQ personnel who will be overseeing this project?

Answer: Yes, primary contact at DEQ is Mr. Larry Brown (Larry.BROWN@state.or.us). Contractors shall direct their questions about this project to Forest Service personnel in lieu of contacting Mr. Brown directly.

PBI-005

Question: Can the contract requirement for Certified Waste Water System Operator be periodic?

Answer: Yes, contractor shall have certified personnel available in accordance with Closure Plan and WPCF permit requirements but onsite monitoring may be periodic as long as contract requirements are still met.

PBI-006

Question: Sewage lagoon decommissioning contractor assumes this is to be dredged and land applied. Do we have an area where this can be land applied?

Answer: DEQ has approved the use of land application through slurry of the solids and liquids onsite. Any other method of land application would have to be approved by DEQ. As a result, proposals shall be tailored for land application through slurry or solids and liquids onsite as well as hauling liner cover material off site for disposal.
