14. DESCRIPTION OF AMENDMENT/MODIFICATION
Project Name and Location: Dale Water-Wastewater System Decommission This amendment addresses the following (see attached addendum)
1. Pre-Bid Inquiries (PBI’s) for Contract Bidding (see attached addendum)
2. Fill-in excel spreadsheet (contractors may utilize the attached and submit with their price).
3. Preproposal Attendee List (see attached addendum)
By signature in Block 15B Contractor releases the Government from all claims relating to this modification. This settlement is complete accord and satisfaction. Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

Pre-Bid Inquiries (PBI’s) for Contract Bidding
PBI-007
Question: May contractors improve access roads up to water tank, river intake, and lagoon areas to perfom work?
Answer: Yes, this work is considered incidental to the completion of bid items.
PBI-008
Question: Are contractors required to haul off vegetation around the lagoons?
Answer: Yes, vegetation within lagoon boundary shall be treated the same as cover material and must be disposed of properly. See General Note #5 on Sheet C301 of project drawings.
PBI-009
Question: What are the finished grade expectations for the lagoon site?
Answer: See Keynote #11 on Sheet C301 and Closure Plan for expectations of final grade.
PBI-010
Question: Fencing that must be removed as part of the contract can this be reused?
Answer: Fencing may be reused if it meets traffic control requirements. Fencing is not to be removed until the lagoon site is secured according to Closure Plan 3.9.5 and Project Drawing decommission schedule.
PBI-011
Question: River intake sheet C403 shows two different fill types for the manhole decommission. Which fill type should we use?
Answer: Sheet C403 Keynote #3 states that the manhole shall be cut 24”in below existing grade and backfill with Class-A backfill and Keynote #2 states that the manhole shall be backfilled with Class-E fill material. Contractors shall fill the manhole with Class-A up to 24” below grade then the top 24” shall be Class-E backfill. See Project Drawing Specifications on G200 for backfill definitions.
PBI-012
Question: Are the water/wastewater lines to be removed or capped in place at the building locations?
Answer: Refer to Project Drawings for instructions on capping of lines.
PBI-013
Question: Trees in the land application area will get slurry on them in the process. Do the trees need to be protected?
Answer: No, the site outlined in the drawing has been cleared for land application. Contractors shall follow BMP’s and Specifications in the contract package.
PBI-014
Question: Water pumped from the wet wells must be removed prior to decommision. Can this water be pumped out onto the ground?
Answer: No, water from the settling tanks and wet wells shall be pumped and transported to the lagoons for land application. See Project Drawing for decommission of wet wells and settling tanks.

Question: What is the load limit of the bridge across the North Fork John Day River at Tollbridge Campground?
Answer: See Load Rating Table below.

PBI-016
Question: What fill material is available for backfill of the lagoons and demolition phases?
Answer: ODOT spoils are available on the compound as well as native soil from ditch cleanings (see project maps)
PBI-017
Question: Can contractor’s cross the river or creek to access parts of the system that require decommissioning?
Answer: Contractors can cross the river but may not put equipment in the water.
PBI-018
Question: How far outside the fenceline can contractors pull material for decommission of the lagoons?
Answer: Contractors are restricted to withing FS property that is just outside the fenceline as shown on the Project Drawings.
PBI-019
Question: Can contractors fill from the nearby river for land application?
Answer: Yes, contractors may pull water from nearby Desolation Creek with prior approval from the Forest Service and proper backflow values and fish screens shall be required.
PBI-020
Question: Is the Forest Service getting additional permits?
Answer: No. See General Note 10
PBI-021
Question: Are electrical components part of this contract?
Answer: Yes, see Project Drawings Note 5 of C301 and Note 7 of C30. In addition, the contractor shall include all electrical demolition in their pricing and work requirements, a separate contract will not be issued.
