About this file
This is a Drug-Free Workplace Certification Form required by Florida Statute 287.087 for the Sidewalk Replacement and Repair - South (WORCS 2) project administered by Hillsborough County Engineering and Operations Department. Vendors bidding on this approximately $10,000,000 sidewalk replacement and repair contract must complete and submit this form as part of their proposal. The work includes removal and construction of new concrete sidewalk segments, repair and rehabilitation of existing sidewalks and curb ramps, ADA compliance grinding and retrofit, incidental irrigation and sprinkler system repairs, tree removal as necessary, temporary construction traffic control, and sodding and site restoration activities.
The Drug-Free Workplace Form requires the offeror to certify compliance with six specific requirements: publishing a workplace drug prohibition statement, informing employees about drug abuse dangers and company policy, distributing the statement to all employees engaged in the contract work, notifying employees of five-day reporting requirements for drug-related convictions, imposing sanctions or requiring participation in rehabilitation programs for violators, and maintaining good faith efforts to sustain a drug-free workplace. The form must be signed by an authorized company representative. Additionally, the contract includes a 9% Small Business Enterprise (SBE) subcontractor participation goal as established in connection with the solicitation.
