About this file
This is a Drug-Free Workplace Certification Form required by the State of Florida for a Countywide Fire Hydrant Replacement contract (WORCS-5) administered by the Hillsborough County Water Resources Department. The form serves as the vendor's formal attestation of compliance with Florida Statute 287.087 requirements. The project involves replacement of fire hydrants, hydrant assemblies, and valves on existing water mains throughout Hillsborough County, with an estimated project value of $1,900,000. The County intends to award one or more contracts to the lowest responsive and responsible bidder(s) meeting specifications, with individual award amounts to be determined during bid evaluation.
The Drug-Free Workplace Form requires the vendor to certify implementation of six specific compliance measures: publishing a workplace drug prohibition statement to employees, informing employees of drug abuse dangers and available assistance programs, distributing the statement to all employees engaged in the contract work, requiring employee notification of drug-related convictions within five days, imposing sanctions or requiring rehabilitation program participation for violations, and maintaining good faith effort to sustain a drug-free workplace. The undersigned authorized representative must sign and date the certification. No Small Business Enterprise (SBE) goal was established for this solicitation.
