Bidders are required to provide estimated delivery dates with their proposals, and suppliers must provide a minimum of three calendar days' written notice if unable to meet delivery schedules. Pricing submissions must be itemized as shown on Exhibit D and are considered all-inclusive, with no additional shipping, surcharges, or taxes to be paid by CDOC; bidders must factor shipping costs into their pricing, and all pricing must remain firm throughout the purchase order term. Bidders must complete and return all required exhibits (A through G) and provide responses to vendor questions including company background, years in business, number of employees, Colorado Secretary of State registration status, compliance certifications, past contract defaults or litigation history, and tax payment status.
